Utilizing a case of Tourette's syndrome as an illustration, this paper seeks to refine the problem of causality in medical illness. In contrast to numerous examples reported in the literature, the patient presented in this communication, a 26-year-old man afflicted with tics, convulsive movements, intractable vomiting, and coprolalia, failed to respond favorably to medication, although he showed marked improvement in the course of psychotherapy.Despite rather transparent psychodynamic connections between his life history on the one hand and the nature and vicissitudes of his symptomatology on the other, it cannot be concluded that the psychological factors alone can account for the clinical picture; at best they may be regarded ascontributory, as opposed tonecessaryorsufficientcauses, a distinction that may be applied to other conditions as well.This report includes a review of some of the literature on Tourette's syndrome, with special emphasis on the case of Dr. Samuel Johnson, who in retrospect appears to have been a likely victim of that unfortunate malady.

The purpose of this research was to investigate what effect an auditory subliminal message, produced by speeding up the rate at which it was recorded, would have upon the imagery and dreams of a group of normal subjects. The auditory subliminal message was produced by speeding up a message that was sung until it could not be consciously understood. This message was mixed with a normal music recording and played to nine subjects in the experimental group. Nine subjects in the control group heard the normal music recording without the subliminal message. Both groups were asked to produce a pretest drawing before the tapes were played, an imagery drawing immediately after the tapes were played, and a dream drawing of any dreams they might have that night.A statistical analysis was performed from data collected on blind ratings given to all the drawings by two art therapists. The results indicated a significant difference between the dream drawings and the imagery drawings of the experimental group and the control group. When the drawings were examined, the effect of the subliminal message could be seen.Based on the statistical data and certain drawings collected in this study, it would appear that the auditory subliminal message did have an effect upon the imagery and dreams of the subjects in the experimental group. The results of this study appear to indicate that the unconscious/preconscious mind is able to perceive a recorded verbal message that cannot be consciously understood at the high rate of speed at which it was recorded.

Patients with chronic pain syndromes are commonly depressed. Chronic pain populations also contain distinct subgroups of personality profiles as defined by theMMPI. To assess the relevance of personality subtype to affective disorder we determined the relationship of psychiatric diagnoses defined by Research Diagnostic Criteria (RDC) toMMPIsubgroups in a sample of hospitalized patients with predominantly chronic low back pain.RDCpsychiatric diagnoses for the sample were major depression (44.2%), minor depression (19.2%), other psychiatric disorder (13.5%), and no mental disorder (21.6%). Patients satisfyingRDCcriteria for major depression were significantly associated with discreteMMPIpersonality subtypes.No other psychiatric diagnoses were significantly associated with distinct personality subgroups. No relationship was observed between personality profile and presence of demonstrable organic etiology for pain. These findings indicate that behavioral and pharmacological interventions directed at depression as well as pain are important in the treatment of chronic pain populations, especially in selected subgroups.

Twenty inpatients suffering from major depressive illness with melancholia were administered the hostility subscale of the Kellner Symptom Questionnaire and Paykel's Clinical Interview for Depression before and after treatment with amitriptyline. A matched control group of normal subjects had the same assessments at two points in time. Hostility decreased and friendliness increased in depressives after amitriptyline; upon recovery, there were no significant differences in hostility between depressed patients and control subjects, whereas such differences were striking during the illness. Patients who had reported losses before onset of illness rated themselves as more friendly than the other depressives; their hostility did not significantly decrease with recovery. The results suggest that hostility improves with the treatment of depression; life events appear to influence the degree of hostility in depressive illness as well as the response to treatment.

The present study was carried out to determine whether children at risk for schizophrenia might exhibit abnormalities in test performance indicative of defective interhemispheric integration and abnormal specialization of language function. Twenty-two high-risk and 22 matched normal children were administered two independent tests of speech comprehension, delivered under monaural and binaural listening conditions, and a standard verbal dichotic listening test. On both comprehension tests the high-risk children exhibited significantly poorer binaural, relative to monaural, recall. On the verbal dichotic test the normal group exhibited a significant right ear advantage whereas for the high-risk group there was no significant difference between right and left ear recall. These results were taken to indicate abnormalities of both interhemispheric integration and language specialization in the high-risk group. Analyses indicate that these two abnormalities may be related in a specific fashion. The implications of these results are discussed within a developmental framework.

A case is reported in which a patient was hospitalized with a severe mixed extrapyramidal symptoms-catatonic reaction after ingestion of three 10-mg tablets of haloperidol that he had purchased on the street, believing them to be Valium and intending to “get high.” The diagnosis was initially missed because the patient and family reported no history of neuroleptic treatment. The syndrome resolved quickly with supportive care. Clinicians must consider this possibly underrecognized syndrome in patients presenting with sudden onset of parkinsonian and catatonic symptoms, particularly when substance abuse may have occurred.
