The sudden surge of reports on multiple personality in the 1970s raises serious questions as to whether multiple personality is the rare phenomenon it has been believed to he, or whether it is a relatively common condition which is misdiagnosed with a high order of frequency. Recent research in ego psychology, particularly into the etiology of the borderline disorders, sheds considerable light on the origins of multiple personality which has traditionally, and probably mistakenly, been considered a wholly hysterical disorder. The author proposes a detailed theory of the genesis of multiple personality, based on the existing ease literature and on contemporary theories of ego development. The issues of authenticity, nosology, diagnosis, personality structure, neuropathy, etiology, and treatment are considered at length.

This is the third repent of a patient with dissociated personality followed for 38 years. There has been a gradual integration of the dissociated states. The most remarkable aspect of the clinical course has been the patient's excellent work record in contrast to a severe rest riction in her social life. The original diagnosis of hysteria was changed to schizophrenia. Evidence is presented to show that the diagnosis of schizophrenia was incorrect. The authors believe that dissociated states occur more frequently than diagnosed, and that the syndrome should be given independent diagnostic status.

Aggressive conduct disorder, defined broadly on the basis of fighting, disobedience, destructiveness, and meanness, was diagnosed in 65 out of 136 boys and 17 of 43 girls consecutively admitted to a psychiatric clinic. Psychotic, brain damaged, and seriously retarded children were excluded from the series. Boys and girls with conduct disorder differed significantly from those with other diagnoses on a number of noncriterion symptoms, particularly those grouped as reactive and egocentric. Boys with conduct disorder were more often involved in antisocial behavior, girls in precocious sexual behavior. The study presents a more detailed clinical picture of the disorder, and suggests that the presence of specific antisocial behavior may be a useful criterion for dividing affected boys into two roughly equal subgroups.

A search of all multiple admissions to the University of Iowa Psychiatric Hospital yielded 20 bipolar patients first admitted during adolescence and 21 bipolar patients first admitted between the ages of 30 and 40 years. Inclusion required the patient to meet criteria for mania on some admission other than the first. Adolescents were as likely as adults to meet criteria for mania and/or depression during their first admission. There were no group differences in distribution of criteria symptoms or in rates of associated symptoms. Likewise, the groups had similar rates of recovery at discharge and subsequent time spent in hospital. The authors conclude that differences between adolescents and adults need not obscure an appropriate diagnosis of mania.

This survey was undertaken in a peasant society without modern psychiatric services in order to assess marriage, reproduction, and parenting functions among 35 mentally ill persons. Among the mentally ill, single persons had earlier onset of mental illness as compared to those who had ever been married. Those who had married and produced children had a high rate of divorce from their spouses and separation from their children following onset of mental illness. Among 12 parents whose onset of mental illness began when their children were still young, five were not living with their children and the other seven exercised little or no parental supervision. In two families, the psychotic parent had beaten her children severely. One infant died of neglect when a psychotic mother refused to surrender child care to others. Mentally ill subjects 18 years or older were less apt to be married than opium addicts in the same population. Infant mortality was greater among the children of mentally ill than in the general population.

The authors report results from a psychiatric study of narcotic addicts using diagnostic criteria from DSM-III and other clinical data. Evidence is presented for a strong relationship between personality disorder and methadone dose, in which patients with schizoid-like disorders had significantly higher stabilization doses than patients with other types of diagnoses. The treatment implications of individual differences in drug and personality interaction are noted, and the findings are discussed in terms of a psychodynamic understanding of addiction

Patients who have the wish to bo admitted and treated repeatedly in hospitals are a fascinating although still poorly understood group. Those who have no apparent organic basis for their complaints are especially interesting and challenging, since they often demand and sometimes receive potentially dangerous somatic treatments such as operations and medications. Such patients receive a wide range of psychiatric diagnoses, such as hysteria, Briquet's syndrome, factitious illness, Munchausen's syndrome, malingering, and schizophrenia. We recently treated a young woman who had been given most of these diagnoses, whose case history presented us with an opportunity to review problems in differential psychiatric diagnosis as applied to this frequently confused and confusing problem area.
