ObjectiveUterine procidentia incarcerated by a vesicle calculi is a rarely encountered condition. The diagnosis and management is reviewed.Study DesignThe unique case of a 74-year-old woman with uterine procidentia incarcerated by a vesicle calculi is described. A medline search for all cases of incarcerated uterus was performed.ResultsA review of the literature revealed only 10 reported cases of uterine prolapse with incarceration. Our patient had an incarcerated uterus and anterior vaginal wall. Despite a total vaginal hysterectomy, the anterior vaginal wall remained irreducible. Cystoscopy confirmed an intravesicle bladder calculus. A vaginal cystotomy was performed to remove the stone.ConclusionVesicle calculi are the most common cause of irreducible prolapse and should be considered in all long-standing cases. Radiologic studies should be used to diagnose urinary calculi preoperatively. Vaginal cystotomy remains the surgical management of choice.

BackgroundIn an effort to decrease the postoperative complications after pelvic exenteration, dura mater allograft was used to cover the denuded pelvic floor.Materials and MethodsBetween 1981 and 1992, dura mater allograft was used to cover the pelvic floor after pelvic exenteration in 24 patients at the Division of Gynecologic Oncology, Jackson Memorial Hospital/University of Miami Medical Center.Results:The incidence of bowel prolapse and pelvic abscess below the graft was zero. A urinary fistula from an infected urinoma developed in one patient. The incidence of enteroperineal fistulae was reduced from 33.3% to 11.1%, when dura mater was covered with a serosal surface, such as omentum or peritoneum.Conclusions:The use of dura mater reduced the incidence of serious complications after pelvic exenteration, such as bowel prolapse and pelvic abscess. The combination of dura mater with a serosal surface, to cover the pelvic floor, reduced the incidence of enteroperineal fistulae.

ObjectiveTo critically examine blood transfusion practice at the time of hysterectomy. Study Design: The authors compared the medical records of patients who had and had not signed a waiver of the use of blood products while undergoing hysterectomy.ResultsThe study group consisted of 93 women (predominantly Jehovah's Witness). There were 186 women in the control group. There was no difference in the study and control patients' ages, preoperative hemoglobin, duration of surgery, operative blood loss, length of hospital stay, or postoperative complications. Hysterectomies were done for abnormal bleeding in 59.1% of the study patients versus 43.4% of the controls(P= 0.01). No study patient received blood, but 10 control patients were transfused(P= 0.03).ConclusionBlood transfusion has been overused at the time of nonradical hysterectomy.

BackgroundThe etiology of pelvic support defects is doubtless complex. Although various factors including connective tissue collagen, striated muscle, and innervation of both striated and smooth muscle have been implicated, lifestyle appears to play an important role.PatientsNine nulliparous airborne trainees with urinary incontinence are reported. All patients reported minimal or no incontinence before airborne training, but had severe urinary incontinence after the rigors of airborne training.ConclusionThis report suggests that pelvic support defects and/or stress urinary incontinence can be precipitated by an isolated traumatic event.
