ObjectiveTo examine if the outcome of sphincter repair is compromised if done in combination with a major urogynecologic procedure. Fecal incontinence combined with urogynecologic disorders is commonly seen in women.MethodsTwenty women who had undergone overlapping sphincter repair combined with a major urogynecologic procedure for urinary incontinence or anterior pelvic organ prolapse were identified from a database, and matched with 20 women from the database who had undergone overlapping sphincter repair alone during the same time period. Preoperative and postoperative data were prospectively collected.ResultsWomen undergoing combination surgery were significantly older than were those having sphincter repair alone (median age, 60 years vs 38 years;P= 0.001) and had a significantly greater median number of daily bowel movements (3 vs 1;P= 0.03). The two groups were similarly matched in all other variables examined. When comparing fecal incontinence scores, both groups showed significant improvement, and there was no difference in improvement in the combination group compared with the sphincter-repair-alone group.ConclusionSphincteroplasty combined with anterior pelvic floor surgery has similar results and complication rates when compared with sphincter repair alone. Combination surgery should be offered to appropriate patients.

ObjectiveReview of the use of various bone anchor systems and their application in urologic and gynecologic procedures, with attention to efficacy and complications.MethodsBone anchoring systems are increasingly used in the surgical treatment of female stress urinary incontinence, postprostatectomy incontinence, and pelvic floor prolapse. Electronic and hand search of the English language literature dealing with bone anchors and their use in the fields of urology and gynecology was conducted.ResultsBone anchor systems show encouraging short-term results and promising longer-term success rates, but concerns regarding complications specific to bone anchors have been raised; osteitis pubis, osteomyelitis, suprapubic pain, and bone anchor dislodging have been reported in the literature.ConclusionThe efficacy and safety of bone anchors in the treatment of SUI should be further evaluated. Long-term results will determine the role of pubic bone anchoring systems in the treatment of patients with SUI and pelvic floor prolapse.

BackgroundPrimary diseases of the seminal vesicle are rare. The advent of transrectal ultrasonography, computed tomography, and magnetic resonance imaging has made the discovery of isolated benign lesions of the seminal vesicles, such as cysts, more frequent.Case ReportAn 80-year-old man with a seminal vesicle cyst was treated by laparoscopic surgical approach.ConclusionThe advantages of laparoscopic surgery include the ease of recovery and short hospital stays. Laparoscopic seminal vesicle excision is relatively simple and safe.

BackroundYolk sac endodermal sinus tumor is the second most common malignant germ-cell tumor of the ovary. Fortunately, it is a rare occurrence and even more unusual in pregnancy. A patient who presented with brachial plexopathy in pregnancy and was found to have a metastatic endodermal sinus tumor is reported.CaseA 34-year-old multigravid woman at 29 weeks gestation presented with fatigue, hemoptysis, supraclavicular adenopathy, and Pancoast tumor. Metastatic endodermal sinus tumor with hepatoid pattern was diagnosed. Due to worsening plexopathy she delivered at 30.5 weeks gestation by classic cesarean section. The infant was normal, without evidence of metastatic disease, but the maternal side of the placenta contained metastatic disease. The patient succumbed to her disease despite aggressive postoperative chemotherapy.ConclusionEndodermal sinus tumor can present in an unusual fashion and unusual symptoms in pregnancy should be investigated.

BackgroundStruma ovarii is a rare ovarian neoplasm that belongs to germ cell tumors of the ovary. Few reports of struma ovarii seen with computed tomography or magnetic resonance imaging exist, because of its extreme rarity.Case ReportA 62-year-old woman presented with acute lower abdominal pain. Pelvic ultrasound showed a predominantly unilocular cystic mass with a small mural cyst. On computed tomograph, the tumor displayed homogeneous high density. Laparotomy revealed a twisted left ovarian tumor and the left hematosalpinx. A total hysterectomy and a bilateral salpingo-oophorectomy were performed.DiscussionMicroscopically, the tumor was composed exclusively of thyroid tissue characterized by diffusely proliferated thyroid follicles and hyperplastic papillary nodules. However, no cellular or nuclear atypia was noted. In addition, the tumor lacked accompanying teratomatous components or the elements of strumal carcinoid. These pathologic findings led to the diagnosis of proliferative pure struma ovarii.ConclusionThis case illustrates that it may be difficult to differentiate struma ovarii composed of a unilocular cyst from serous cystadenoma, ovarian cyst, or paraovarian cyst on the basis of computed tomography.
