ObjectiveThe study was designed to compare the prognostic ability of DNA aneuploidy and S phase % by flow cytometry to that of tumor size and stage to assess prognosis in granulosa cell tumors of the ovary.Study DesignA chart review of 21 patients treated for granulosa cell tumors collected data on age, FIGO stage at diagnosis, tumor size, presence of disease, treatment, and survival. Available archival specimens were prepared for flow cytometry by a modified Hedley method. By obtaining DNA histograms and calculation of DNA index, ploidy and S phase % were assigned. Prognostic indicators were then statistically compared to survival.ResultsEarly stage rumors (I and II) were associated with relatively better disease-free survival. Tumor size > 10 cm did not indicate a poor prognosis. Of 12 adequate flow cytometry cases, aneuploidy and S phase % elevation were equal between disease-free and persistent or progressive disease groups.ConclusionEarly stage granulosa cell tumors have an excellent 5-year survival. However, other indicators to predict late recurrence are unproven. Large prospective trials with long-term follow-up are needed before flow cytometry can be adequately evaluated as a prognostic indicator for this neoplasm.

ObjectiveTo review the historical and current practices of the pull-through procedure for middle and lower rectal cancers.MethodsAnalysis of the outcome and evaluation of physiologic function of Parks' coloanal anastomosis, the Bacon pull-through, and the Cutait operation.DiscussionBefore the advent of the circular stapler, the pull-through procedure was practiced primarily for reconstruction of inaccessible lesions in the lower pelvis. Compared with the other procedures for removal of the rectum, the operation has been proven to be equally as effective for cancer cure and postoperative morbidity. The functional results vary according to the technique employed. Physiologic data from the Bacon pull-through operation indicate that because of the loss of the reservoir function and the failure of reflex inhibition of the internal sphincter, the patients have to initiate the act of defecation with an enema. Conclusion: The pull-through procedure is a valuable sphincter-saving operation. With several chemoradiation protocols currently active, it is projected that the procedure will see a resurgence.

BackgroundThe gynecologic surgeon is rarely involved in the acute presentation of intraabdominal hemorrhage where it is not the result of an extrauterine gestation. Leiomyoma uteri are more commonly associated with chronic hemorrhage when symptoms do occur. The authors report a case of severe hemorrhagic shock associated with a pediculated leiomyoma uteri.CaseA 45-year-old woman with a history of leiomyoma uteri presented to the emergency room in profound shock. Massive intraabdominal hemorrhage associated with the rupture of an aberrant uterine vein is reported. A review of similar cases is also included. Conclusion: Although benign, leiomyomata uteri may be associated with severe life threatening complications. Nonpregnancy related gynecologic etiologies of intraabdominal hemorrhage should be kept in mind when evaluating the patient with hemorrhagic shock. As conservative management of leiomyoma uteri becomes increasingly popular the gynecologic surgeon must be aware of the potential complications of nonsurgical management.

摘要:
ObjectiveTo describe an unusual uterovaginal anomaly consisting of a vertical and lateral defect.DesignCase report depicting this rare anomaly.InterventionTo clearly define the anatomy and establish a patent outflow tract, an exploratory laparotomy was performed. The patient was found to have bicornuate uterus with an uterine septum extending to the cervix. An opening in this septum between the cavities was observed. Additionally a longitudinal and a transverse vaginal septum was noted.ResultsTo date the patient continues to have regular periods.ConclusionsIn cases of multiple fusion defects, an exploratory laparotomy may be necessary to establish the diagnosis.
