Background:Laterally extended pelvic exenteration with or without brachytherapy of the tumor bed is a novel therapeutic option from which selected patients with pelvic recurrences of cervical cancer can benefit.Methods:Patients with pelvic recurrences of cervical cancer entered into a prospective study that investigated relapse patterns, prognostic factors, and the role of extended radical treatment. Management was based on the diagnostic findings and the patients' option for therapy. From 1990‐1997, 138 patients with recurrent cervical cancer in the pelvis participated in the study.Results:Recurrent tumor size, recurrence‐free interval, and extended radical treatment with potential local control were the strongest independent prognostic factors in the multivariate regression analysis. Tumor location was not a prognostic factor. The survival rate for patients with central disease treated with exenteration alone did not differ from patients with side wall disease who underwent laterally extended pelvic exenteration with or without brachytherapy of the tumor bed.Conclusions:Patients who experienced pelvic recurrences of cervical cancer with tumors ≤ 5 cm in diameter and detected > 5 months after primary therapy benefited from extended radical treatment with potential local control, irrespective of tumor location. Because the overall prognosis for patients with pelvic tumor recurrences remains poor, means of prevention should be given a high priority.

Background:Laparoscopic techniques were first described at the turn of the 20th century, but were not used regularly until recently, with the development of laparoscopic cholecystectomy. The authors present current applications of laparoscopy for the management of urologic disease in the adult patient.Methods:Review of the literature to identify laparoscopic procedures that have made an impact in adult urology was performed. Each procedure was assessed for general technique, outcomes compared with open surgery, and controversies surrounding their current use.Discussion:Early in their development, many laparoscopic applications were practiced with general dissatisfaction because of the lack of consistent cases needed to maintain laparoscopic skills. The ongoing efforts of several laparoscopic centers have improved techniques, and laparoscopic procedures are used regularly for various diagnostic, ablative, and reconstructive procedures. Long‐term follow‐up evaluation data show that many laparoscopic procedures result in improved postoperative recovery times and outcomes equivalent to open surgical procedures.Conclusion:Laparoscopy is well established in the urologic field. Laparoscopic techniques are used for adrenalectomy, pyeloplasty, varicocele repair, bladder suspension, nephrectomy and lymph node evaluation for genitourinary disease. However, the ongoing effort to improve techniques and patient outcomes is necessary.

&NA;In 1910, an American‐born English homeopathist, Dr. Hawley Harvey Crippen, was charged with the murder of his wife by the use of scopolamine. He was captured after a transatlantic chase and was tried, convicted, and executed in England. This was the first case in British legal history of murder by the use of scopolamine.
