Forty-one normal full-term infants were fed from birth, during the first 4.5 months of life, different diets based on two formulas similar in rough composition but basically different, the one being constituted only of cow milk protein and the other of soy protein. From each of the two formulas two different dilutions were prepared so that all diets supplied about 100 kcal/kg/day, but, respectively. 2.0 and 4.0 g/kg/day of cow milk protein and 2.0 and 5.0 g/kg/day of soy protein. After 4.5 months, growth in weight, length, and head circumference was normal and very similar in all infants. Gammaglobulin, immunoglobulin, transferrin, and some complement fractions (particularly C'3, C'1 INA, C'3 PA) were lower in infants receiving soy protein diets than in those receiving cow milk protein diets, and, within each type of diet, in those infants receiving lower amounts of protein. In particular, values of infants receiving 5.0 g/kg/day of soy protein were roughly comparable to those of infants receiving 2.0 g/kg/day of cow milk protein. B lymphocyte markers and reactivity did not show significant differences among the various groups. T lymphocyte markers and reactivity showed an impairment in soy protein diets (and in particular in 2.0 g/kg/day protein diets) with respect to cow milk diets. It was also observed that morbidity (mainly infections of upper respiratory tract) was higher in those infants who took soybean protein and in those who took lesser amount of protein.

The hydrolysis of bovine α-lactalbumin, β-lactoglobulin, and casein by duodenal juice from 20 infants (18 with normal exocrine pancreatic function, 2 with pancreatic insufficiency), aged 3–19 months was studiedin vitrowith the aid of electroimmunoassay and sodium dodecyl sulfate–poly-acrylamide gel electrophoresis. The results from the two methods were almost identical. Duodenal juice from infants with cow's milk protein intolerance (7), celiac disease (5), and unclassified gastrointestinal disorder (6) had the same capacity to hydrolyze the milk proteins. No hydrolysis occurred in the two patients with pancreatic insufficiency. The hydrolyzing capacity was not correlated with age in the actual age group. The hydrolysis of the milk proteins occurred at a considerably slower rate when the proteins were crude, as in cow's milk, adapted, or unadapted formula, than when they were in a purified form. About 1 mg of purified α-lactalbumin or β-lactoglobulin, and about 30 mg of purified casein could be hydrolyzed per milliliter duodenal juice per minute. Corresponding figures for the hydrolysis of the various proteins in cow's milk were 0.03, 0.12, and 16.1 mg/ml duodenal juice/min. Preincubation (60 min) with gastric aspirate after adjusting pH to 4–5 did not change the results. In conclusion, the duodenal juice from infants with normal exocrine pancreatic function has a great ability to hydrolyze casein. Corresponding hydrolytic capacity for α-lactalbumin or β-lactoglobulin is considerably lower.

The serum levels of five acute phase proteins (APP) were measured in 18 children with Crohn's Disease (CD) or ulcerative colitis (UC) and in two control groups. The levels of C-reactive protein, α1-acid glycoprotein, α1-antitrypsin, C9, and Factor B were significantly raised in patients with CD and UC with good separation from controls, but they were not entirely reliable used as screening tests unless used in combination. The levels of APP were monitored for periods varying from 18 to 28 months in each patient and found to reflect the disease activity in both CD and UC. On seven occasions the APP levels did not match the clinically assessed disease activity, but when the serum levels were related to outcome of the disease. C-reactive protein was found to be elevated—whether or not there were symptoms of the disease—in all patients who later had a relapse, while normal values were found in those who had a long remission. These results suggest that the estimation of C-reactive protein is of prognostic value and that its measurement is particularly useful in children with mild symptoms in whom disease activity and prognosis are difficult to assess.

Human milk calcium (Ca) and phosphate (P) content from mothers of 10 term and 15 preterm infants was evaluated during the first month of lactation. Ionized Ca of preterm milk was higher than term milk on day 14, 6, 70 ± 0.26 (mean ± SEM) vs. 5.47 ± 0.09 mg/dl, and on day 30, 6.35 ± 0.14 vs. 5.67 ± 0.13 mg/dl (ttest, p < 0.05). Total Ca and P levels of term and preterm milk were not different, and decreased during the first month of lactation. Nursing preterm infants had higher serum P than term infants on day 14 and 30. Serum Ca and 25-OH D values were not different between nursing preterm and term infants or between the lactating mothers.

In a group of 7− to 12-year-old children without manifest clinical iron deficiency, urinary excretion of free norepinephrine was found to have substantial positive correlation with measure of total iron-binding capacity and negative correlation with serum ferritin. Results support earlier reports of an inverse contingency between iron status and urinary norepinephrine level as observed with severe iron lack, findings consistent with speculation that monoamine oxidase activity may be modified by iron availability.
