A before and after study was carried out amongst staff of 10 wards of a county hospital before and after the introduction of a 12‐hour shift system for nurses. The purpose was to investigate the impact of the shift system on job satisfaction. Some 320 nurses covering all qualified and unqualified grades were surveyed using a standard job satisfaction attitude scale. It was found that under the 12‐hour shift both intrinsic and extrinsic factors of job satisfaction had been detrimentally affected. Considerable dissatisfaction was expressed about hours of work, conditions of work and the impact of the shift on domestic and social arrangements. Organizational aspects of neonatal care were documented in a large scale national survey: these included the provision of cots, the nursing establishment, medical staffing and the use of nurses qualified in the specialty. Fifty‐six unidentified neonatal units in England were contacted and information requested on staffing arrangements, policies and facilities. To gain an accurate picture, four representative units were selected in each health region: a regional centre, a sub‐regional centre and two district units. Comparisons were made using data on cot provision, the nursing establishment set, the numbers of trained nurses and medical staffing in the target units. Large inter‐unit differences were found and in many units the levels of staffing were found to be at lower levels than those recommended. The relationship between the nursing establishment and numbers of designated cots was examined using different models of staffing based on successive recommendations. The more recent the recommendation, the greater the discrepancy between the establishment in operation and that recommended. Large inter‐unit variation was also found in the proportion of staff qualified in the specialty, ranging from 0% to 92% of qualified nurses in individual units. Overall 1544 83% trained nurses were working in the study units, of whom 820 53% were qualified in the specialty. Significant differences were found between the types of unit: regional and sub‐regional units had a higher ‘Qualified in Speciality’ rate 50% and 49% than district units 32%. These figures are lower than the 70% level recommended by the English National Board for Nursing, Midwifery and Health Visiting, but the overall figure is an improvement on the levels recorded in 1981 and that reported for paediatric nurs

摘要:
When the economical systems in the health care field are becoming more like those in ‘the market for goods and services’ it is important that the patients' need of nursing care as well as the costs for the nursing care can be documented in a systematic and reliable way. One way of doing that is to use a patient classification system. In this article the most used patient classification system in Sweden–the Zebra system–will be presented. The Zebra system makes it possible both to describe the individual patients' dependency level and to calculate the patients' requirements of nursing care both in staffing terms and costs, per month, per year, per patient stay, per diagnosis or DRG. It gives possibilities for managers to analyze the patient distribution in different categories of care during different periods of time, or if the patients' need of nursing care can explain the staffing requirements during a certain period, or if the distribution of days in different categories of care are related to age groups or diagnosis

摘要:
It is evident that the increased use of computer technology in health care settings is having an impact on the nursing profession. This paper reviews the literature examining the attitudes of nurses to computerization, concluding that, although computers are being increasingly incorporated into the nurse's working environment, the literature lacks adequate research support of the attitudes held by psychiatric nurses toward computerization. A move toward computerization, as with any innovation, can be greatly facilitated if nurses respond to the change in a positive manner. Accordingly, a greater understanding of psychiatric nurses' attitudes toward computerization could be an integral part of the process of computerization within psychiatric settings.

摘要:
The recent profound changes in health care policy and the management and organization of the National Health Service have had a major impact upon nursing. Indeed, the development of the new management culture has challenged the organizational and management structure of nursing. This paper explores the role of nursing leadership in relation to the management of care delivery. The need for nursing leadership, its struggle for emergence and its effectiveness are discussed. The author concludes that for nursing to survive in the reformed health services strong leadership is essential. To achieve this the profession must move away from its traditional power base and refocus on clinical practice and the management of nursing rather than the management of nurses.
