The devastating transfusion reaction that can occur after the infusion of blood contaminated with bacteria has prompted blood banks to adopt practices designed to minimize the possibility of bacterial contamination. There are four recognized sources of contamination of blood collected for transfusion: 1) prior contamination of the bag or the anticoagulant solution; 2) airborne contamination of the needle; 3) inadequate skin preparation including coring of the skin; and 4) bacteremia in the donor.27The use of plastic collection bags and specially designed needles, and the recognition of the importance of aseptic technique have drastically reduced the incidence of serious transfusion reactions due to contamination. The history obtained from the donor prior to phlebotomy also eliminates many donors who could have bacteria in their blood at the time of donation. Improved collection techniques and the deferral of potential high risk donors have made serious reactions from contaminated blood a rare occurrance.9

Auto anti‐U of unusual serologic characteristics is described in a Caucasian patient with autoimmune hemolytic anemia associated with chronic lymphocytic leukemia. The direct antiglobulin test was strongly positive (3+) with broad spectrum and anticomplement reagents. Auto anti‐U was demonstrated only when the serum was acidified to pH 6.5 and anticomplement reagent was used in the antiglobulin test. Multiple transfusions of U positive red blood cells had clinically decreased survival manifested by jaundice and return of hemoglobin to pretransfusion levels within days. Samples from 168 lots of immune serum globulin, tetanus immune globulin, and RhD(D) immune globulin produced by seven American manufacturers during the period 1973–1977 were analyzed for anti‐A and anti‐B content by saline and antiglobulin titration. There was appreciable variation among manufacturers, but between 1973 and 1975 all products showed a significant decrease in alloantibody titer. This trend did not continue during the interval 1975–1977; the titers of most manufacturers' products remained near the 1975 level. Very little has been published to indicate the quantity of fibrinogen in cryoprecipitates. We assayed 88 preparations from five blood banks for factor VIII (AHF) and fibrinogen to assess whether the AHF assay can predict the fibrinogen content. Cryoprecipitate was considered to be consistent with FDA standards with 80 units of factor VIII/bag (40% yield from 200 ml plasma). Fibrinogen was considered adequate if 200 mg were recovered (40% yield, 200 ml plasma, normal range 150–350 mg/dl). The mean AHF was 145 units/bag and fibrinogen was 266 mg/bag. There was a wide range among bags. In 64/88 bags, the fibrinogen and AHF were concordant, but in 24/88 bags the results were discordant. Although it appears safe to conclude that a bag of cryoprecipitate will average 250 mg fibrinogen, adequate control may require separate assays for fibrinoge

Platelet concentrates were prepared from blood collected in CPD, CPD‐A1, and CPD‐A3. CPD‐A1 contains 1.25 times as much glucose as does CPD, and sufficient adenine to provide a final concentration of 0.25 mM; CPD‐A3 contains twice as much glucose as does CPD, and sufficient adenine to provide a final concentration of 0.5 mM. The consumption of glucose and the production of lactate and pyruvate in the platelet concentrates during room temperature storage was not influenced by glucose or adenine concentration. There was no difference in the rates of fall of the pH of platelets collected in the three preservatives. However, the number of contaminating white blood cells had a significant effect on these parameters. In a previous study we have reported that stored erythrocytes have an augmented tendency to agglomeration in 0.25 M sucrose, at pH 7.2. In the present study we determined the role of protein coating of the erythrocyte membrane and the effect of pH in the agglomeration phenomenon. Erythrocytes, which did not agglomerate and had no protein coating when fresh, agglomerated with storage; although no membrane gamma globulin was detectable, their surface contained fibrinogen. When fresh erythrocytes were coated with albumin, gamma globulin, fibrinogen or hemoglobin, they did not agglomerate. The incubation of fresh washed erythrocytes in ACD or PBS at pH 5.5, in the presence or absence of plasma, induced agglomeration, and the cells reacted with antibody to fibrinogen. Fresh cells from patients with immune disorders, including multiple myeloma, immune‐hemolytic anemia, and rheumatoid arthritis, did not agglomerate. Erythrocytes of these patients were stored in the presence and absence of their plasma, at different buffer systems. Agglomeration after storage was independent of the protein level and the pH of the system appeared to be the major factor. Agglomeration also occurred after several weeks of storage at constant pH of 7.1 and absence of proteins. The findings are consistent with the hypothesis that traces of fibrinogen are adsorbed to circulating erythrocytes, but are only accessible to the antibody reagent after metabolic membrane alterations had occurred after storage. Platelet concentrates prepared by a discontinuous flow centrifugal technique (Haemonetics) were examined for evidence of bacterial contamination and subsequent growth from 3 to 11 days after collection. Of the 126 platelet concentrates examined, 4 revealed bacterial growth. However, the growth patterns indicated contamination during the microbiologic manipulations rather than contamination of the units during preparation. These studies indicate that platelet concentrates prepared using the Haemonetics Blood Processor can be safely transfused for up to three days after collection if stored at 4 to 6 C.
