Using both a radioactive chromium and an automated differential agglutination (ADA) technic donor erythrocyte survivals were measured in 19 subjects. The data were analyzed to determine the presence of age‐dependent (linear) and random (exponential) destruction. The erythrocyte lifespan was calculated from the rate of linear destruction. In all cases it was possible to estimate the erythrocyte lifespan with the ADA technic. Significant random destruction of red blood cells was observed in nine patients. With the51chromium technic it was possible to estimate the erythrocyte lifespan in only ten cases; these values were similar to those obtained with the ADA technic. It was not possible to determine the erythrocyte lifespan in the remaining nine cases; the best estimates were too large. Normal hemoglobin function depends on adequate erythrocyte levels of 2,3‐diphosphoglycerate (2,3‐DPG), a compound which is poorly maintained in acid‐citrate‐dextrose (ACD). Since 2,3‐DPG is better maintained in citrate‐phosphate‐dextrose (CPD) and this preservative has a higher pH (5.5) than ACD (pH = 5.0), these preservatives were prepared at each pH and studied. The CPD preservatives (pH 5.0, 5.5) had similar amounts of phosphate so the differences between them, obtained by altering the buffer ratio, should relate to pH. The ACD solutions (pH 5.0, 5.5) contained no phosphate. Hemoglobin function, expressed as P50(the Po2at 50 per cent oxygenation, an inverse but direct measure of oxygen affinity), and 2,3‐DPG were better maintained in ACD and CPD of pH 5.5. The lower pH (5.0) preservatives, whether ACD or CPD, showed rapidly declining hemoglobin function and 2,3‐DPG levels. We have studied the response of platelets to a standardized osmotic stress following storage at either 4 C or 22 C. Platelets that had been stored at 22 C for up to 72 hours retained their ability to recover from hypotonic swelling, while those that had been stored at 4 C showed a rapid decline in their ability to recover from hypotonic stress.

Cultures for bacterial growth were taken from platelet concentrates prepared at ambient temperature and stored for 24 to 96 hours at 30 C. A total of 120 cultures were taken at varying times from 40 platelet concentrates. The collecting system was not broken. No organisms were isolated which indicates that the hazard to the patient from bacterial contamination is not enhanced by this method of preparation.

Three patients who received incompatible transfusions are reported. Two developed a generalized bleeding diathesis and died. Clotting studies showed disseminated intravascular coagulation. Fibrinogen and fibrin split products were found in extremely high amounts as a result of secondary activation of the fibrinolytic system following the intravascular coagulation episode. One patient was treated with heparin with cessation of the hemorrhage. Prompt treatment with heparin should be the therapy of choice for the bleeding tendency which accompanies hemolytic reactions.

Trauma and abruptio placentae with delivery of a stillborn infant at 36 weeks' gestation was followed by a fetomaternal transfusion of ABO incompatible erythrocytes, and maternal hemoglobinemia and hemoglobinuria. Fetal hemoglobin was demonstrated in the maternal plasma and urine by electrophoresis and alkali denaturation. The mother had a benign postpartum course.

Plasma fibrinogen determinations were performed before and after cryoprecipitate infusion in two hemophiliacs who received periodic infusions on prophylactic basis. The mean increases in plasma fibrinogen were 6.5 mg 100 ml per unit infused in one patient and 5.1 in the other. This corresponds to an administered dose of 205 mg/unit of cryoprecipitate. Cryoprecipitate offers the advantage of decreased hepatitis risk, but the disadvantage of wide variation in fibrinogen content.

One hundred platelet concentrates were stored at 25 C and cultured for bacterial growth on four successive days. No bacterial contamination was detected which suggests that platelet concentrates, prepared in a closed system, are sterile and remain so under these conditions.
