Human erythrocytes preserved by a low‐glycerol, rapid‐freeze method maintained normal lipid concentrations after storage at ‐196 C for three to 146 weeks. In contrast to erythrocytes stored in ACD at 4 C, the frozen cells exhibited no tendency to develop progressive losses of total lipid weight, cholesterol, or lipid phosphorus. Relative distribution of individual phospholipids was similar to that of fresh control specimens. No streaking or other evidence of products of oxidative degradation was observed in thin‐layer chromatograms, and assays for peroxide formation in lipid extracts were unaffected by prolonged storage of the frozen cells. Dimethylacetamide (DMAC) was used as cryoprotective agent for the preservation of platelets in the frozen state. The addition of dextrose to the platelet suspension greatly increased the cryoprotective effects of this agent. Rat platelets preserved with 5 per cent DMAC and 5 per cent dextrose showed a higher percentage of morphologically intact cells and higher clot retraction‐promoting activity than platelets frozen in other concentrations of these two agents. The infusion of these platelets into thrombocytopenic rats resulted in platelet‐count increments greater than those observed after administration of platelets frozen in 5 per cent dimethylsulfoxide (DMSO) and 5 per cent dextrose. Ten commercial “broad‐spectrum” antiglobulin sera, which had satisfied NIH standards, were evaluated and compared with six antiglobulin sera of varying specificities prepared in the authors' laboratories. The majority of the commercial antisera had adequate anti‐IgG, although four were definitely weaker and failed to detect weak IgG antibodies. Most of the commercial antiglobulin sera had inadequate anticomplement, eight failing to react with cells strongly sensitized with complement, from patients with hemolytic anemia. The greater affinity of hemoglobin for carbon monoxide than for oxygen can lead to increased carboxyhemoglobin in blood donors who smoke. The presence of carboxyhemoglobin has an adverse effect on the oxygen dissociation curve resulting in a decreased release of oxygen to the tissues. When this is present, along with the progressive decrease in oxygen‐carrying capacity resulting from storage, bank blood will have an even greater loss in ability to provide oxygen to the tissues of a recipient. Carbon monoxide can be displaced from hemoglobin by oxygen when there is room air exchange by donor respiration. The rate of this exchange can be accelerated by taking advantage of the natural gas laws and using a higher concentration of oxygen for respiration or by increasing ventilatory exchange. Volunteers in the present study raised their carboxyhemoglobin levels by heavy smoking and had the rate of decrease in carboxyhemoglobin tested either after breathing pure oxygen or after exercise‐induced hyperventilation. The decreases in carboxyhemoglobin levels were accelerated by either method. Donors who had not smoked showed low carboxyhemoglobin levels which did not decrease after exercise. Thus, blood banks could minimize carboxyhemoglobin levels in collected blood by restricting the smoking of donors, by not collecting blood for at least 2 to 4 hours after smoking, or by using oxygen breathing or exercise‐induced hyperventilation to accelerate the donors excretion of carbon mon

Continuous‐flow centrifugation of blood from normal donors with the NCI‐IBM Continuous Flow Blood Cell Separator can be used safely and effectively to collect large quantities of normal granulocytes, lymphocytes, and platelets. Comparison with single‐unit leukapheresis demonstrated a threefold greater collection of granulocytes and a fivefold greater collection of lymphocytes per hour of leukapheresis with continuous flow centrifugation. Granulocyte collections can be enhanced by giving the donors etiocholanolone or cortisone prior to leukapheresis. A procurement program is described which is designed to provide blood from volunteer donors for about 10,000 transfusions (10,000 pints) a year. An automated antihuman globulin test has been performed by continuous flow analysis. Cell washing in acidic low ionic medium achieved sedimentation of aggregated cells and removal of supernatant fluid without centrifugation. These cells were subsequently disaggregated with hypertonic antihuman globulin serum, but human antibody‐coated cells remained aggregated and were removed. Remaining red blood cells were hemolyzed, and their hemoglobin concentration was measured colorimetrically.
