摘要:
Medical loss ratios indicate the proportion of total operating revenue spent for health care. Managed care organizations monitor these ratios carefully to keep them from surpassing predetermined targets. The same is true of administrative expense ratios. Low medical loss and high administrative expense ratios have become political hot buttons for advocacy groups unhappy with managed care's growing displacement for fee-for-service medicine.

Clearly, subacute care providers who are JCAHO accredited have a competitive edge over those who are not. As Medicare and Medicaid continue to shift to managed care plans and as employers' groups become more familiar with subacute care providers, those that can get accredited within 2 years will be in an excellent position to capture a large share of the market. The challenge, then, is how to prepare for a successful accreditation process.

摘要:
An inexpensive method of determining the staffing requirements for a labor and delivery unit is described. Data were collected hourly by an independent observer to determine the workload by hour of day in this unit. By matching RN staffing with the pattern of activity, implementing cost training and developing guidelines to respond to volume changes, the cost per delivery has increased only 16% since 1988.

Downsizing and tighter budgets are making it difficult to find cost-effective courses to prepare nurses for specialty area patient care. This is particularly true of small community hospitals, where overhead costs of group classes are high and participant numbers are low. To address this concern, a self-paced, multimedia teaching program with a nontraditional teaching/learning format was developed.

Job satisfaction of the nursing staff at a 1,000-bed neuro-psychiatric veterans administration (VA) facility was assessed, with results similar to those of other studies. The greatest source of stress was nurse/supervisor and nurse/physician interpersonal conflict. This article reports on a further study of this factor.
