Despite its growing popularity, the medical loss ratio (MLR) does not measure the quality of care that a managed health plan provides to its members. This ratio also does not fulfill other purposes for which it is used. Conclusions based on MLRs alone may be wrong and result in flawed public policy.

This study examines the span of control, the scope of responsibilities and the perception of effectiveness of first-line managers (nurse and non-nurse) in two complex health care organizations. Results suggested first-line nurse managers had larger spans of control, fewer assistants and made less money than non-nurse first-line managers.

Advanced practice nurses and staff nurse skin-care team members provided the necessary expertise and information to integrate AHCPR recomemendations in a protocol for patients at risk for skin impairment. Regular practice monitoring and education regarding skin management have proven to be successful strategies. A skin-impairment management protocol is provided.
