摘要:
Abstract:The JCAHO advises that staffing changes, with or without restructuring, should include a framework for improving patient care. This article explores such a framework, as well as addressing common questions about staffing. Both the JCAHO and Nursing Management's editors urge you, the readers, to submit your questions (see address at the end of the column).[Nurs Manage 1997:28(11):16,20]

摘要:
Abstract:Technologic forces are transforming both the nursing profession and nursing education. How will these changes affect nurses? What is at stake? As society moves to an information economy, one's value will be determined by the ability to access and use information (knowledge).[Nurs Manage 1997:28(11):26–27]

摘要:
Abstract:The Balanced Budget Act of 1997 calls for significant changes in the Medicaid and Medicare programs. Three measures help subacute care providers to meet the changes this act poses: (1) understanding a prospective payment system; (2) developing a financial model; and (3) implementing efficient systems that decrease cost.[Nurs Manage 1997:28(11):28–29]

摘要:
Abstract:The types of health plans that may contract with Medicare have been expanded significantly. Medicare beneficiaries will have to make a determined effort to decipher the assortment of choices they now face. Health plans are required to take prescribed steps to educate Medicare beneficiaries about their choices and must contribute to costs the federal government incurs for related outreach efforts.[Nurs Manage 1997:28(11):30–31,33]

摘要:
Abstract:Point of care testing is changing for the better the way we practice medicine. Whether it is an anesthesiologist doing a blood gas in the operating room, a nurse measuring serum glucose in the home or on the unit, or an intensive care nurse testing by the bedside before calling an intensivist, newer instrumentation provides for a limited number of quality tests to instantly add value to the critical decision-making process of modern staff. This movement is a true paradigm shift that will reduce unnecessary testing, provide timely measurement of critical values and avoid unnecessary and catastrophic mistakes.[Nurs Manage 1997:28(11):34–36]

摘要:
Abstract:Balancing quality health care with the pressures of managed care and restructuring requires monitoring and measuring outcomes. The “nursing report card” organizes data in an informative manner, allowing nursing's contributions to patient outcomes to be assessed and reported to all customers in an ongoing fashion.[Nurs Manage 1997:28(11):38,40–41]

摘要:
Abstract:Individuals and facilities convicted of health care fraud are likely to be excluded from participating in any federal health program. Knowingly disregarding the truth by submitting false claiins is one type of fraud that is punishable by law—from a stiff fine to a lengthy imprisonment. Understanding the types of fraud in the health care context helps prevent abuse and Hability.[Nurs Manage 1997:28(11):42,44–45]

摘要:
Abstract:Although many facilities believe turning over some patient care to unlicensed caregivers will cut costs, results show otherwise. The greater the rate of change—patient acuity—the greater the need for an all-RN staff. Skill mix, length of stay and salary costs among five teaching hospitals measure the cost effects.[Nurs Manage 1997:28(11):47–48]

摘要:
Abstract:Adopting an open-minded attitude and the willingness to try new ideas are effective ways to cope with change and help reshape health care delivery. The willingness to let go of the relatively stable, nonthreatening situation and embrace a new, ambiguous environment will promote personal and professional possibilities. Keys to organizational success include: accept change; redefine the business of the organization; encourage high-performing, flexible individuals; and foster a sense of team learning and community. The changes in health care delivery, the complexity of patient care and the blending roles all provide new options for nurses.[Nurs Manage 1997:28(11):50–54]
