Thirteen cases of idiopathic common peroneal nerve palsy were reviewed and the clinical presentation and course evaluated. Incomplete nerve palsy of unknown aetiology has a good prognosis and all the patients in this group made a full recovery without surgical intervention. Those presenting with a clinically and electrophysiologically complete lesion made no recovery, despite surgical intervention in two cases. It is suggested that these lesions can safely be treated conservatively as long as there is no identifiable cause.

All patients examined at The National Hospital for Nervous Diseases, Queen Square, from 1983 to 1987 with radiological evidence of meningiomas involving both optic nerves within the orbits have been reviewed. Their clinical and X-ray computed tomography (CT) features are different from those of patients with unilateral tumours, and they appear to form a separate group, probably representing optic nerve involvement by a primarily retro-orbital meningioma.

Continuous electromyographic (EMG) activity and responses to electrical stimulation of the facial nerve were monitored intraoperatively in 35 patients undergoing primary removal of acoustic neuroma. The rate of anatomical and immediate functional preservation of the facial nerve of these patients was compared with a matched population where a facial nerve stimulator and monitoring of facial muscle contraction by the anaesthetist only was employed. Overall, 94% (n = 33) of facial nerves were preserved anatomically in patients with continuous EMG monitoring, compared with 64% (n = 89) of patients (n = 139) without continuous monitoring. Amongst the 33 cases with anatomical facial nerve preservation, 64% (n = 21) had immediate functional preservation. Immediate functional preservation was only achieved in 39% (n = 35) of anatomically preserved facial nerves (n = 89) where conventional monitoring was used.

Laser angioplasty is used to vaporize atheroma in limb arteries. The present investigation was undertaken to determine the nature, size and quantity of particulate matter generated, and the appearance of the intima of a carotid artery model after laser treatment. Particulate matter was not significantly increased following treatment of 39 autopsy carotid bifurcations, but superficial plaques were vaporized and the surface of fibrous lesions glazed. It is anticipated that atheromatous carotid vessels would be rendered less thromobogenic and the risk of cerebral embolism reduced later angioplasty.

Transcranial Doppler sonography (TCD) flow velocities and cerebral blood flow (CBF) measurements were evaluated in 14 patients who had suffered a major aneurysmal subarachnoid haemorrahge (SAH). Cerebrovascular reactivity to hypocapnia, was evaluated simultaneously by the two methods. The measurements were performed under general anaesthesia preoperatively, within 72 hours after the bleed, during normocapnia and hypocapnia. There was poor correlation between absolute values of hemispheric CBF and corresponding TCD mean flow velocity. Controlled hyperventilation was associated with a significant decrease in CBF as well as TCD flow velocity (p<0.001). In terms of reactivity indices the correlation between the two methods was poor and not significant (r=0.33,p= 0.09). The principal differences between the methods are discussed as well as the application of TCD in the evaluation of cerebrovascular reactivity.

A bipolar electrode has been designed to assess the possibility of locating accurately the electrode tip position within a nerve pathway prior to its thermal coagulation without having recourse to a wake-up procedure. By electrically stimulating each of the peripheral divisions of the trigeminal nerve and recording the evoked activity pre-ganglionically from the electrode, using an averaging computer, a clear triphasic response is recorded. The amplitude of this wave is largest when the electrode is within the stimulated division, thereby confirming electrically the anatomical location. By incorporating a thermocouple in the tip of the electrode a precisely controlled thermocoagulation can then be achieved using a radiofrequency generator. A preliminary series of 12 patients with trigeminal neuralgia requiring operative treatment were assessed in this study. In eight cases a clear response was obtained, in two the signal-to-noise ratio was too small, in one stimulus artefact obscured the response and in one a blood vessel was hit.

The effects of high velocity missiles are described. A series of cases of craniocerebral tangential gunshot wounds over a 6 year period is presented with unsuspected cerebral contusion shown by CT. Tangential wounds are defined and classified into four groups. The associated brain damage, often unsuspected prior to CT, and in some cases without radiographic changes, of the four groups is compared. The short- and long-term sequelae and the extent of cerebral damage belie the innocent appearance of the scalp wound. This influences the management of this unusual type of head injury.

One-session bilateral craniotomies for radical clipping of multiple aneurysms have been performed in 14 patients after subarachnoid haemorrhage. Bilateral small or medium sized aneurysms in pre-operatively good-grade Hunt and Hess (HH)I-III) patients (nine cases) were clipped without mortality or morbidity. Four poor-grade patients (HH IV at admission, improving to HH III pre-operatively) were treated, with unfavourable outcome in one patient. Another poorgrade patient (HH IV at surgery) with bilateral large middle cerebral artery aneurysms, a large intracerebral haematoma and a pontine angle meningioma survived with dysphasia and short-memory dysfunction. A slack brain at surgery and a short operation time justify one-session bilateral craniotomies also in the early post-bleeding period.

Statistical modelling was used to analyse clinical features and the Ki-67 proliferation index in a study of 77 cases of glioblastoma multiforme. Relative youth, frontal tumour site and treatment with external beam radiation had an important positive influence on survival. The pre-operative Karnovsky score and the presence of necrosis were not related to outcome. Despite the attractive hypothesis that rapidly proliferating tumours might be associated with a worse prognosis compared with slowly proliferating lesions, the Ki-67 index in this study offered no prognostic information even when individual sites were considered separately. The use of this form of computer modelling and its role in analysis of prognostic data is discussed.
