The specific aim of this study was to determine the accuracy with which nursing home staff can use descriptive scales to evaluate the dietary intake of residents. The dietary intake of 50 randomly selected subjects was observed for 3 consecutive days and the mean intake of calories, protein and vitamin C was calculated. Licensed practical nurses (LPN) and nurse aides (NA) rated the adequacy of each residents' nutritional intake using 3-scales (Gosnell's 1973 general nutrition scale; protein and vitamin C intake scales). The percent agreement between staff rating of diet intake and the mean diet intake ranged from 22% to 44%. Reliability between LPN and NA ratings was low. The validity of the scales as determined by percent agreement between actual observed/calculated intake and rating scales was also less than acceptable. Interestingly, staff tended to rate diet intake as less favorable than it actually was observed to be. This error in rating may be useful since staff are more likely to attend to the nutritional needs of residents whose intake is deemed to be inadequate rather than adequate.

The effect of dining location on dietary intake was evaluated for the institutionalized elderly. Eating with a group provided a significantly greater percentage of the Recommended Dietary Allowance for vitamin E while dining in rooms provided diets which were significantly more nutrient dense for vitamin D, riboflavin, calcium, and phosphorus. Regardless of where residents ate, at least nine of the 16 nutrients studied were present in amounts less than predicted. From a dietary standpoint, perhaps greater attention to what nursing home residents eat is more important than where food is eaten.

Modifications of formulas for three baked products accomplished significant increases in vitamin A, calcium, thiamine, and riboflavin; but calories were reduced in only one product. Modifications were economically practical for home preparation of two products, but the third was suited to commercial production. Fifty elderly individuals liked the original and modified nutritious cookies, whereas fifty college students neither liked nor disliked them. Both the elderly and college students preferred commercial whole wheat to white bread, and both groups liked the modified whole wheat bread. Both age groups liked the control and modified muffins, but the elderly from two locations differed in their ratings.

This study was conducted to determine the contribution of the Title III meal program to the overall nutrient intake of participants. There were 69 participants in the study with a mean age of 74 years. Overall nutrient intake of the participants was adequate with mean intakes of all nutrients exceek 67% of the RDA. Black subjects consumed more carbohydrates, fiber, calcium, iron, thiamine, niacin, riboflavin, and vitamin C at lunch than did white subjects; however, these differences were due to different consumption patterns reported for "home" lunches rather than differences in lunches eaten at the meal site. Lunches supplied at least 33% of the total daily nutrient intake regardless of whether it was a meal program lunch or a home lunch.

Herein describes a method for estimating the energy needs of the elderly amputee using the relationship between body surface area (BSA) and basal metabolic rate (BMR). First the preamputation BSA is determined by graphing the preamputation weight and height on a BSA nomogram. Next, the BSA of the amputated extremity is subtracted from the preamputation BSA to give the postamputation BSA. This value is then used in Wilmore's BSA equation to determine the postamputation BMR. The total energy expenditure including the energy expenditure of ambulation can be estimated by one of two methods. The first method applies the increased energy expenditure of ambulation for amputees to all non-basal activities. The second requires the actual distance ambulated per day. The latter provides a "truer" estimate of energy needs since ambulation is the major energy expenditure of elderly amputees.
