The CRNI Certification Examination consists of 200 multiple-choice questions covering the nine core content areas of infusion nursing: Technology and Clinical Applications, Fluid and Electrolyte Balance, Pharmacology, Infection Control, Pediatrics, Transfusion Therapy, Antineoplastic Therapy, Parenteral Nutrition, and Quality Assurance. The review questions provided below are modeled on the CRNI exam and are intended to help exam candidates test their knowledge of infusion therapy practice. This special section is a regular addition to theJournal of Infusion Nursing, with each edition focusing on a single content area.

摘要:
The use of vancomycin continues to be prevalent in all clinical settings. However, many questions persist about infusion techniques. According to theInfusion Nursing Standards of Practice,peripheral catheters are not the best choice for infusing this drug because of its pH. The key to reducing risk of peripheral phlebitis and extravasation injury is choosing a more appropriate vascular access device. Many healthcare providers correlate systemic side effects with the infusion rate and concentration, although many reports cannot support this correlation. New technologies of vascular access and infusion controlling devices are changing old, established practices. This update provides an examination of the current literature on all aspects of infusing vancomycin and monitoring patients.

Venous catheters are placed for the purpose of infusion, and, in the case of central vascular devices, for drawing blood specimens for laboratory analysis. Little scientific research exists on optimal methods to obtain blood samples from catheters, and clinicians use a variety of non-proven techniques. This article includes an overview of multiple techniques, review of research studies and publications, and recommendations for current practice.

Infusion therapy specialists are recognized for their technical expertise and their knowledge of the anticipated reactions and adverse effects of many complicated therapies. But when asked the rationale for intravenous fluid selection, many infusion nurses can only shrug their shoulders. Although intravenous fluid selection may seem a complex principle, it is really quite simple. Once the flow of body fluids among fluid compartments is understood, it is easy to follow the intended path of IV fluids. But infusion specialists should be cautious of the manner in which IV fluids are categorized prior to infusion (isotonic, hypotonic, and hypertonic) because these are not always consistent with how they react in the body.

Low molecular weight heparins (LMWH) are replacing unfractionated heparin (UH) as safe and effective agents for the prevention and treatment of thromboembolism. Although LMWH offer many advantages over UH, usage is less clearly defined in certain special populations, including renal dysfunction, obesity and pregnancy. This article will briefly review the pharmacology of LMWH and discuss usage in these special populations.

摘要:
Enhancing medication safety continues to be a critical focus area in healthcare. Barriers to improving safety include a culture of blaming individuals, underreporting of errors, and neglecting to analyze and improve systems. This article describes why medication safety should be linked with a non-punitive reporting system. Drug safety issues specific to home infusion care are identified, with practical recommendations for enhancing safety and reducing medication errors. Included in this discussion is the sentinel event alert for infusion equipment free flow and the JCAHO Patient Safety Goals for 2003.

摘要:
Chronic inflammatory diseases, such as rheumatoid arthritis, psoriasis, and Crohn’s disease, affect a large segment of the population in the United States. Recent research suggests that, in many inflammatory conditions, an inflammatory protein or cytokine, specifically tumor necrosis factor-alpha (TNF-α), appears to play an important role. Medications designed to treat these chronic inflammatory diseases, in addition to alleviating its symptoms, have become important therapeutic alternatives. This article will discuss specific Immune Mediated Inflammatory Disorders (I.M.I.D.), their treatment and management, as well as the current clinical application and potential future role of TNF-α antagonists as a common therapeutic modality.
