摘要:
Changes in chemotherapy protocols have influenced the risk and rate of secondary malignancies in high-risk populations. The alkylating agents, topoisomerase inhibitors, and anthracycline agents pose the highest risk of initiating carcinogenesis. Normal cells that are especially sensitive to chemotherapy and most likely to begin carcinogenesis include those of the bone marrow, hair follicles, and the epithelial cells of the gastrointestinal tract. Thus, the development of secondary hematologic cancers such as leukemia and lymphoma pose the greatest risk to adult and childhood cancer survivors. Lifelong surveillance is recommended.

摘要:
This article presents the results of a prevalence research study conducted at Winchester Medical Center in Winchester, VA. This research study assessed the impact of implementing a dedicated infusion therapy team to provide central vascular catheter care and administration of total parental nutrition (TPN) on the rate of nosocomial bacteremia. Data were collected from monthly infection control reports and retrospective chart reviews conducted by infusion nurses. Nosocomial bacteremia rates are compared before and after implementation of infusion therapy central venous catheter and TPN administration intervention.

摘要:
Human blood group antigens are unique, inherited polymorphisms on the extracellular surface of red blood cells. They have been used as genetically discrete markers of human polymorphism since the discovery of the ABO system in 1900. Since then, many blood group antigens have been identified, the genes cloned, and their biological significance elucidated. Blood group antigens and antibodies play an important role in Transfusion Medicine. In addition, blood groups have provided anthropologists with a tool to study polymorphism in the different peoples across the world and provided geneticists with inherited markers to understand complex mechanisms of linkage and disease inheritance.

Rheumatoid arthritis (RA) is an immunologically mediated disorder characterized by progressive joint destruction that leads to significant impairment of functioning and quality of life. Its signs and symptoms vary, depending on disease activity. The goals of therapy in patients with RA include a reduction of symptoms, inhibition of structural damage, and improvement in physical function. The paradigm for managing RA calls for adopting a three-pronged strategy that addresses the different aspects of the disease. Infliximab, a biologic response modifier, has been shown to be an effective and safe therapy in patients with RA. Infliximab, in combination with oral or subcutaneous methotrexate, is administered intravenously, most commonly in an office-based setting. As with any intravenously administered protein, infusion-related adverse events have been reported with infliximab; however, such events are infrequent, and slowing the rate of the infusion may reduce their likelihood of occurring. These adverse events can generally be managed easily. Nurses who administer infliximab should have a thorough understanding of the product to prevent or manage adverse events. In this way, they can help ensure the safe delivery of the agent and optimize patient outcomes.

摘要:
Loss of peripheral intravenous (PIV) catheter patency is a common problem in the clinical setting. A survey of all private and public hospitals in a metropolitan city in Australia was conducted to identify practices related to maintaining patency of PIV catheters. Eighty-three hospitals were surveyed and responses were obtained from all of them. Normal saline was the most commonly used solution for flushing. The frequency for flushing ranged from 4 to 12 hours, and the volume of the irrigant used ranged from 2 to 10 mL in those hospitals where flushing is routine practice. Policy documents were available from only a few hospitals to validate the practice reported. Significant diversity in practices was documented among healthcare organizations.
