The CRNI Certification Examination consists of 200 multiple-choice questions covering the nine core content areas of infusion nursing: Technology and Clinical Applications, Fluid and Electrolyte Balance, Pharmacology, Infection Control, Pediatrics, Transfusion Therapy, Antineoplastic Therapy, Parenteral Nutrition, and Quality Assurance. The review questions provided below are modeled on the CRNI exam and are intended to help exam candidates test their knowledge of infusion therapy practice. This special section is a regular addition to theJournal of Infusion Nursing, with each edition focusing on a single content area.

The need to find safer blood products escalated in July 1982 with the first report of AIDS in a person with hemophilia. By 1985, approximately 10,000 people with hemophilia were infected with HIV1and manufacturers were providing products that had been treated for viral inactivation. Approximately 8000 people with hemophilia have died in the AIDS epidemic, and approximately 2172 are surviving with HIV chronicity.2The number of deaths are an enormous loss in a tightly knit and highly organized hemophiliac community whose activism brought about the Ricky Ray Hemophilia Relief Act. These 8000 lives will not be forgotten. The search for safer products, such as recombinant DNA technology products, is ongoing. Of persons using recombinant products in the United States, 61.3% have hemophilia A and 54.8% have hemophilia B. To meet the coagulation needs of the hemophilia population, it can be expected that innovative delivery models will emerge in the United States and globally.

Infusion resource teams are comprised of nurses specially trained and experienced in infusion therapy. Our multidisciplinary team provides clinical, educational, and research support to a 1000-bed Canadian tertiary hospital. To characterize the infusion resource nurse service, 789 recorded consults for 250 patients during a 12-month period study were reviewed. Noncritical medicine and surgical wards accounted for a similar number of consults, with the highest volume (31% of total consults) being generated by the general and vascular surgery wards. Vein status was visible and either “fair” or “good” in approximately half of all consults, but 39% of consults were visible and “poor.” Most consults (81% of total) resulted in the initiation of peripheral intravenous catheters into an area of nonflexion in an upper extremity and successful peripheral catheter initiations were accomplished in 96% of all cases. Our multidisciplinary infusion program approach to vascular access support appears to be a well-utilized and an effective resource for this hospital.

Blood transfusion and component therapy are an integral part of treatment for patients with certain life-threatening illnesses. Any benefit derived from transfusion must be weighed against the potential risk for complications.Patients who are chronically transfused present a unique challenge to practitioners. The presentation will focus on alloimmunization to red blood cell antigens, platelet refractoriness, transfusion-associated graft-versus-host disease, and cytomegalovirus infection. Prevention or management of these complications also will be discussed.

Benchmarking data has validated that patients with planned vascular access device (VAD) placement have fewer device placements, less difficulty with device insertions, fewer venipunctures, earlier assessment for placement of central VADs, and shorter hospital stays. This article will discuss VAD program planning, early assessment for VAD selection, and benchmarking of program data used to achieve positive infusion-related outcomes.
