摘要:
Lidocaine, a drug commonly used via intravenous infusion for the treatment of cardiac arrhythmias, is frequently effective but has the hazard of potential toxicity. Avoidance of toxicity and better control of therapy are shown to be markedly enhanced by a new method of rapidly measuring blood levels of lidocaine by enzyme immunoassay. Examples of the use of this technique for drug monitoring in the settings of suspected toxicity, prospective therapy, dose response assessment, and the study of pharmacokinetics are presented.

We present a method for the analysis of antidysrhythmic drugs [procainamide, acecainide (NAPA®), lidocaine, quinidine, disopyramide,N-desisopropyl disopyramide, and propranolol] in serum. The drugs, together with an internal standard, are extracted from 0.2–1.0 ml of serum, separated on an octyl-bonded reversed-phase column using a mobile phase consisting of acetonitrile/phosphate buffer, and monitored by either ultraviolet or fluorescence spectrophotometry. The proposed method offers good reproducibility, sensitivity, linearity, and accuracy. Of more than 50 drugs and metabolites tested for possible interference, only diazepam, flurazepam, and theN-oxide metabolite of quinidine interfere with quinidine analysis, while meperidine coelutes with disopyramide. However, diazepam and flurazepam do not interfere with quinidine analysis with fluorescence detection.

摘要:
A single high-recovery extraction process is described for rapid gas-liquid chromatographic (GLC) determination of valproic acid (VPA). The drug is extracted from acidified plasma into an equal volume (0.2 ml) of chloroform. Evaporation is avoided by extraction into a chloroform bed, from which an aliquot is analysed by isothermal GLC employing a column packed with 10% DEGS. The reproducibility and accuracy of the method was good. In 100 plasma samples from patients, there was an excellent correlation (r= 0.98) between VPA levels determined concomitantly by this method and the technique of Kupferberg. Single plasma samples can be analysed in 20 min, and 40 samples can easily be analysed daily by a single technician. Ethosuximide concentrations can also be determined simultaneously. The method is suitable for emergency and routine use in drug monitoring laboratories.

摘要:
We describe potential interference of procainamide with our high pressure liquid chromatography procedure for assaying theophylline (similar to that given by Orcutt et al.). In participating in the AACC-TDM quality-control program, we discovered procainamide interference on a column dedicated to theophylline and acetaminophen serum analyses. Following attempts at rejuvenation, this 7-month-old column was replaced with a new one, which gave base-line separation of theophylline and procainamide with no difficulty. We recommend close attention to the aging characteristics of columns and participation in a quality-control program.
