AbstractPostmortem examination of the brain is essential for the diagnosis of Alzheimer's disease. In the context of a study on the natural history of the condition, the authors describe their experience in obtaining consent for this procedure. AbstractConventional techniques for postmortem acquisition of brains have several shortcomings for biochemical research. The authors describe the development of a technique for acquisition of brains by rapid autopsy and the practical details and problems of setting up the research programme. The techniques were established for the investigation of excitatory amino acid neurotransmitters.

AbstractReminiscence groups are being increasingly used as a ‘therapeutic’ activity with elderly people suffering from dementia. A report on two separate reminiscence groups carried out in two different day centres is presented. The settings are described along with the aims, methods and content of the two groups. The study compared the effect of reminiscence with the effect of alternative group activities on elderly people's interaction in the two separate settings using an observational methodology. The interaction pattern of a staff member in each centre was also observed. The impact of the reminiscence groups in the two day centres was quite different, illustrating the need for careful definition of environmental factors in evaluating such approaches. Where there was previously little interaction during an alternative group activity, reminiscence produced a dramatic change in the behaviour of the staff member and elderly people observed. In an initially ‘richer’ environment, reminiscence did not effect a change in either staff or elderly people. AbstractThe mental and physical capacity of all persons living permanently in a nursing home in Bergen were estimated and their neuroleptic drug consumption and duration of stay were registered. AbstractFew studies have compared methods of case identification in dementia in detail. In this epidemiological study of cognitive function and dementia, scales and clinical diagnoses were compared in 365 women aged between 70 and 79. The women were interviewed using CAMDEX (Cambridge Examination for Mental Disorders of the Elderly), which includes established and new scales of cognitive function and behaviour. In addition, the information/orientation subscale of the Clifton Assessment Procedure for the Elderly was recorded. The proportions of the population identified by various recommended cutpoints for these scales were calculated. Sensitivities and specificities for these were compared, using CAMDEX clinical diagnosis as the standard. Large differences were found in these according to both the cutpoint set and the level of dementia examined. Each subject was rated by her own general practitioner and a random 10% of the sample was seen using the Geriatric Mental State Examination in addition to CAMDEX. The different diagnoses were compared and considerable variation found.

AbstractTetrahydroaminoacridine (THA), a centrally acting anticholinesterase, was used in a two‐month, double‐blind, placebo‐controlled crossover trial to treat 10 patients meeting DSM‐III‐R criteria for dementia of the Alzheimer type. Eight patients continued to take THA for a further three months. Nausea was a frequent side‐effect. Five patients developed abnormal liver function tests; liver biopsies showed evidence of liver cell necrosis in three patients, a granulomatous reaction in one, and one recovered after reduction of THA dosage. During the trail, patients as a group showed a significantly better performance on one of 10 memory tests when taking THA as compared to placebo. One patient showed a marked clinical improvement, six showed some improvement, and three patients showed no improvement with the active treatment. AbstractAggressive behaviour is a serious care problem for families looking after patients with dementia. The aim of this study was to investigate, in a community sample of patients with dementia, the nature of aggressive behaviour and the settings in which it occurred. Thirty‐seven subjects known to be aggressive were studied. The results show that the acts of aggression can be usefully classified on the basis of the circumstances in which they occur. It is argued that such a classification is likely to be of more value in understanding aetiology than is a classification based on the nature of the acts themselve
