AbstractThe literature on grief is reviewed with particular reference to old age. The characteristics of normal and abnormal grief are discussed and as far as possible a comparison between older and younger adults is made. The results are inconclusive but suggest that grief is a similar process in all adults but may be less malign in the over 65s. Whatever the differences, grief is a serious problem with a definite associated morbidity and mortality.

AbstractThe majority of nursing home patients in the United States have dementia or behavioral problems yet the outcome of care they receive for those disorders is unknown. We compared the outcomes of Alzheimer's disease patients residing on a dementia unit with other Alzheimer's patients in the same nursing home over a one‐year period. AbstractAll residents in a home for elderly válidos (able) in Spain were interviewed and 101 completed the study. Non‐medical interviewers adminstered the GHQ‐28 items and either Zung's Self‐Rating Depression Scale (SDS) (N=69) or the Geriatric Depression Rating Scale (GDS) (N=31). The residents were also examined blind by psychiatrists standardized in the use of the Clinical Interview Scale (CIS), who also administered the ‘Mini‐Examen Cognoscitivo’, the Spanish version of the Mini‐Mental State Examination (MMSE), and diagnosed the identified ‘cases’ according to ICD‐9 criteria. Twenty‐one residents (20.8%) were considered to be ‘cases’ on the basis of the CIS; 14 (13.9%) were diagnosed as depressed and only four (4%) were diagnosed as being demented, although the mean age of the sample was 81.9 years (SD=5.6). Most ‘cases’; were of mild intensity and the differences with ‘non‐cases’; seemed quantitative rather than qualitative. The GHQ‐28 correctly identified 81.5% of ‘cases’; with a cutting score of 5/6 (sensitivity 70%, specificity 84.4%) and 77.4% of ‘cases’; with a cutting score of 4/5 (sensitivity 85%, specificity 75.3%), suggesting a discriminative power almost as good as in adult individuals. AbstractMany writers have commented on the frequency with which old people express fear of falls but this fear has not been quantified. As part of an epidemiological study of falls in the elderly living at home, 203 people over 75 years of age were questioned about falls experienced in the previous 12 months, fear of falling and symptoms of dizziness, and assessments were made of their mood, cognitive function, physical state, dependence and mobility. Fear of falling affected the same proportion of subjects as falls themselves (42.4%) and although there was an association between fear of falling and having experienced falls, a third of non‐fallers limited their activity because they were frightened of falling. Falls, fear of falling and subjective dizziness were strongly associated with anxiety and depression. Subdivision of fallers into outside and inside fallers or those who had fallen because of trips and ‘non‐trip’ fallers produced groups which differed in their physical frailty, mobility and dependence, but even the otherwise healthy and independent outside fallers and ‘trip’ fallers had high anxiety and depression scores. AbstractThis community‐based epidemiological study is concerned with relationships between social and environmental factors and depression in a Finnish population aged 60 years or over. A greater proportion of depressed than non‐depressed men had retired and did not work at all. The occurrence of depression in both sexes was related to retirement because of sickness rather than age, a small number of rooms in their homes, low numbers of hobbies, lack of intimate friendships, and the occurrence of many long‐standing and current social stress factors. Positive associations were also found in the men between the occurrence of depression and moving due to poor health, widowed or divorced marital status, being frequently alone and the fear of death, while a positive relationship was found between the occurrence of depression and low social participation in the women. In addition, the depressed women felt that elderly people were not appreciated. Also, the depressed women felt their relationships with their spouse, children, daughters‐in‐law and friends to be more distant than did their non‐depressed counterparts. AbstractClinical recovery, cognitive function, and ward behavior indicative of organic brain syndrome were assessed in 12 elderly, depressed patients before, and approximately one week after, treatment with electroconvulsive therapy (ECT). There was a statistically significant improvement in depression, and ward behavior, whereas cognitive function was unchanged. These findings support and extend prior studies which indicate that ECT in the treatment of depression in the elderly is not associated with functionally significant decrements in cognitive performance.

AbstractA stratified random subsample of 100 older adults from long‐term participants of the Second Duke Longitudinal Study was interviewed concerning how they coped with three stressful event periods. Responses to open‐ended coping questions were categorized as religious or non‐religious in nature. Mean scores on 16 personality traits were compared between religious copers. Few differences were observed in personality scores between these groups. Compared with non‐religious copers, religious copers scored lower on factor E (less aggressive or hostile, more humble, submissive); there was also a tendency towards being higher on factor G (more responsible, conscientious, and moralistic). Over a six‐year period between wave 1 and wave 4, stability or change in personality was not related to coping style.
