AbstractMost elderly people, including the majority of those with dementia, live in their own homes, but a significant minority live in residential homes, nursing homes and hospitals. While a great deal of research has been carried out into the experiences of residents and staff in these latter environments, it is not at all clear what contribution the design of these environments makes to their lives. The physical environment is often treated as a confounding variable in evaluations; and theoretical frameworks are frequently too general in nature to suggest what role design might play. Moreover, these frameworks typically do not acknowledge the phenomenon of architectural determinism, even though it has important consequences for research designs. Alternative approaches, which attempt to be more specific in dealing with design, and which are not deterministic, are discussed. The practical importance of key concepts such as privacy and home is also highlighted.

AbstractEarly morning anxiety was reported as a symptom in 32.1% of 131 elderly and middle‐aged patients hospitalized for depression one to two years following hospitalization. Two‐thirds of those patients complaining of early morning anxiety also complained of anxiety at other times during the day. The demographic and system profile of subjects complaining of early morning anxiety did not differ on a variety of indices from subjects complaining of anxiety throughtou the day at follow‐up, except that early morning anxiety was associated with a less severe symptom profile at follow‐up. These results suggest that early morning anxiety is a less severe presentation of mixed depression/anxiety in patients who do not recover completely from an episode of major depression, regardless

AbstractThe investigation studied the effect of intravenous administration of a dopamine blocking agent (metoclopramide) on serum prolactin levels in patients with senile dementia of Alzheimer type (SDAT) and in non‐demented elderly controls. The response to metoclopramide was greater in the demented patients than in the controls, suggesting that dementia is associated with dysfunction in the tubero‐infundibular pathway which controls prolactin secretion. AbstractThe reliability of the DSM‐III diagnostic system was assessed in a medical long‐term care facility population that was recently seen in psychiatric consultation. Six standard rating scales were used to determine interrater agreement for perceiving psychopathology prior to selecting diagnoses. The findings were that six major diagnostic groups could be accurately distinguished from one another: no disorder; organic mental disorders; affective disorders; paranoid disorders; schizophrenia and psychotic disorders not elsewhere classified; and a mixed group of other diagnoses that occurred in low frequency in this study population. AbstractEight agitated residents from an Alzheimer's unit of a 500‐bed skilled nursing home facility were observed over a period of two months. A behavior‐mapping instrument yielded information about agitated behaviors, and the physical and social environment in which they occurred. Trained observers rated each resident's behavior for three minutes of each hour of the 24‐hour day. The agitated behaviors which occurred most frequently were: making strange noises (babble, cry, scream, inappropriate laughter), constant requests for attention (repeat words, ask questions, ask for help), and repetitious mannerisms. The amount and types of agitated behaviors varied greatly across subjects and time. Some of the subjects exhibited behaviors with uniformly high frequencies during all their waking hours. While two of the eight residents appeared to manifest a sundown syndrome, four residents manifested agitation significantly more frequently during the morning hours, and the data of two residents did not show any clear pattern. AbstractOne thousand and twenty‐three subjects in four different types of statutory accommodation were surveyed during a two‐week period using the Survey version of the Clifton Assessment Procedure for the Elderly. The types of accommodation were conventional residential accommodation (CRA), elderly mentally infirm homes (EMI), geriatric wards (GER), and psychogeriatric wards (PG), all within a specific geographic area. AbstractEight hundred and seventy‐one permanent residents in residential homes (Part III), EMI homes, geriatric hospitals and psychogeriatric wards were examined over a two‐week period and their cognitive and behavioural functions recorded using a reliable and valid questionnaire (CAPE Survey).
