AbstractPositron emission tomography (PET) is a technique which enables direct measurement of cerebral metabolism to be made. The procedure has exciting possibilities for the investigation of brain metabolism but there are limitations and problem which need to be overcome. A review is presented of the current state of knowledge of PET in relation to dementia, the techniques used, the practicalities of performing a scan and the limitations involved.

AbstractThe authors describe relationships between age and ratings of signs and symptoms in 40 manic inpatients. Soon after admission, some psychopathologic features were less prominent in older patients. In older patients, after two weeks of treatment with lithium and neuroleptics, there was greater residual total psychopathology, higher ratings of some clinical features, and less change in total psychopathology and some clinical features. Older patients were hospitalized longer. Ageing may alter manic psychopathology and treatment response. The authors emphasize the need for prospective studies that include geriatric patients.

AbstractSymptoms and signs that in terms of severity differentiate major and atypical depression from dysthymic disorder were investigated in depressed Finns aged 60 years or over. The overall symptomatology of major depression was not significantly more severe than that of dysthymic disorder, although some symptoms, viz. sadness, paranoid symptoms, loss of interest in work and activities, loss of weight and depersonalization, were more severe in major depressive patients than in dysthymic patients. The overall symptomatology of atypical depression was less severe than that of dysthymic disorder. Sadness, psychic anxiety, loss of interest in work and activities, somatic anxiety, general somatic symptoms, strength of diurnal variation of symptoms, gastrointestinal symptoms and suicidal ideas were less severe in atypical depressive patients than in dysthymic patients but loss of insight was more severe in atypical depressive patients. Of objective signs, loss of weight was more common in major depressive patients than in dysthymic patients. Sad expression and crying were less common in atypical depressive patients than in dysthymic patients.

AbstractA correlative neuropathological‐clinical study was undertaken in order to analyse the possible clinical effects of the selective incomplete white‐matter infarctions (SIWI) frequently found in dementia of Alzheimer's type (DAT). The cases studied represent DAT with and without concomitant SIWI, non‐Alzheimer dementia with SIWI and cerebrovascular dementia, including multi‐infarct dementia and Binswanger's disease. The structural grey and white matter changes were examined and the clinical symptoms and signs were analysed. The occurrence of cardiovascular disease and abnormal blood pressure was studied as well. The results show that cardiovascular disease and systemic hypotension occur frequently in cases with SIWI and they are considered of pathogenetic importance for SIWI. Symptoms and signs of a non‐focal cerebrovascular/ischaemic type, mainly vertigo, fainting and a fluctuating course, are associated with SIWI. AbstractProblem drinking and patterns of alcohol consumption are examined as part of a follow‐up study of 1070 elderly subjects randomly selected from Liverpool general practitioner lists. Using a community version of the Geriatric Mental State Examination, the prevalence of current drinking problems was found to be 9.4/1000 subjects aged 65 and over. At follow‐up three years later, one‐third were drinking moderately and a similar proportion whose health had seriously deteriorated were abstinent. Detailed examination of drinking patterns reveals a decline with age in the number of subjects drinking regularly. This decline is accompanied by a trend towards reduction in alcohol consumption with age which is less marked for women. AbstractA survey of the degree of mental impairment, physical disability and behavioural problems was undertaken in residents in psychogeriatric continuing care beds at Friern Hospital (n=96) and Islington Local Authority old people's homes (OPHs) (n = 104). High levels of dependency were identified, particularly in those subjects with a clinical diagnosis of dementia, who constituted 80% of the hospital sample and 43% in OPHs. Precipitants of admission differed between the hospital and OPH groups (incontinence, aggression and wanderingvspoor self‐care and living alone), both for the total sample and for those subjects with a clinical diagnosis of dementia. Subjects with dementia in the two settings, however, showed similarly high levels of dependency and behavioural problems. AbstractThirty‐six female inpatients with a clinical diagnosis of senile dementia, Alzheimer type, were entered into a doubleblind withdrawal of their established thioridazine. Over a four‐week period there were no significant differences between the two study groups in terms of cognitive function, behaviour or physical state. There was no evidence of a withdrawal phenomenon and the use of replacement medication was at a similar low level in the two groups. Although significant differences were not established in terms of side‐effect, there was a trend for greater reduction of these for the group in whom treatment was stopped. These data indicated that careful, controlled withdrawal of established thioridazine treatment is a safe procedure which may be useful in circumstances where the contribution of medication is un
