AbstractProfiles of mental and physical health status and reports of contact with primary care and personal social services were obtained from 979 individuals aged 65 years and over randomly selected from the Nottinghamshire Family Practitioner Committee's records. On the basis of personal disturbance scores derived from the Symptoms of Anxiety and Depression scale (SAD) administered at the time of interview, respondents were classified as psychiatrically ‘normal’, ‘borderline’ or ‘ill’. Those classified as psychiatrically ‘ill’ were significantly more likely to report recent contact with both general practitioners and certain personal social services than were those classified as psychiatrically ‘normal’. Intermediate levels of contact were found among those classified as borderline (between normal and ill). The prescription and use of psychotropic drugs showed a similar mental health gradient. While SAD scores were positively and significantly related to the number of reported physical health problems, the overall correlation between mental and physical health scales was modest (rho = 0.41,p<0.001). These results indicate that while mental health problems are clearly influential in determining family doctor consultations among the elderly, a substantial number of emotionally disturbed elderly people do not consult their general practitioners during the currenc

AbstractThe construction and validation of a brief instrument for assessing social support in the elderly is described. The Index of Social Support (ISS) has nine items and provides separate measures of a person's perceived availability of support and his or her satisfaction with it. The two scales are shown to have high internal consistencies (coefficient alphas of 0.84 and 0.81) and the score totals discriminated significantly between non‐depressed elderly people and elderly people who had suffered a recent onset of depression. Principal components analysis showed that the perceived availability of support subscale provides a single factor accounting for 41% of the variance. AbstractPatients referred for liaison consultation to a Department of Psychiatry for the Elderly were studied over 12 months. One hundred and twelve patients were referred by hospital doctors and most of these were seen on consultations within the hospital. These patients had more physical problems than those referred from the community over that period. The absence of a formal psychiatric diagnosis does not mean that psychiatric skills cannot be useful in managing a patient. Many elderly physically ill people present complex problems and the old age psychiatry service can play a valuable role in providing liaison consultations to other departments within the general hospital.

AbstractTwenty‐eight depressed patients receiving amoxapine with ages ranging from 21 to 68 years (male 12, female 16) were studied in order to clarify the age effect on plasma levels of amoxapine and its major active metabolite, 8‐OH amoxapine. Plasma amoxapine levels were determined by both radioreceptor assay (RRA) and gas liquid chromatography (GLC). Plasma levels of 8‐OH amoxapine were also determined by GLC.A highly significant correlation was found for plasma levels of amoxapine determined by RRA and GLC (r= 0.88,p<0.001). There was no significant correlation between the amoxapine daily dose (mg/kg) and its plasma levels determined either by RRA or GLC. The ratios of plasma amoxapine level by RRA and GLC to daily dose significantly increased with advancing age (p<0.05) and a significant positive correlation was also found between the ratio of plasma level of 8–OH amoxapine to daily dose and the age of the patients (p<0.01), suggesting that the plasma 8‐OH amoxapine levels were more strongly affected by age than the parent drug. These results indicate that the metabolism of amoxapine in the liver decreases with age as well as that of other tricyclic antidepressants reported previously. AbstractPlasma cholinesterase activities were measured in a group of elderly patients and controls. Total cholinesterase activity was found to be decreased to a just significant extent in those suffering from endogenous depression when compared to normal controls. There was a highly significant decrease in cholinesterase activity in the plasma of patients suffering from senile dementia of Alzheimer type (SDAT) but not in those suffering from multi‐infarct dementia, neurotic depression or chronic schizophrenia. There was a just significant correlation between total cholinesterase and acetylcholinesterase activity in the plasma of patients suffering from SDAT and the degree of dementia. AbstractIn a controlled double‐blind study, sulpiride 100–200 mg daily or placebo was administered over a five‐month period to 22 and 24 depressive elderly outpatients. The majority of the patinets in both groups were diagnosed as suffering from relatively mild degrees of depression, i.e. from dysthymic disorder (chronic depression) or atypical depression. According to the Hamilton Rating Scale for Depression, the average symptomatology decreased from 19.7 to 15.7 points in the sulpiride group and from 22.8 to 17.0 points in the placebo group. The scores of three patients in the sulpiride group and those of four patients in the placebo group decreased by 50% or more. The corresponding figures for a decrease of 25–49% were six and nine, respectively. The effect of sulpiride on depressive symptoms did not differ from the effect of placebo. AbstractA case of progressive supranuclear palsy in a 69‐year‐old female is reported. Initially the patient presented to the pscyhiatric services with prominent paranoid delusions. Later, when dementia became more evident, neurological examination revealed typical features of progressive supranuclear palsy, including complete failure of upward gaze, facial bradykinesia, slow gait and bilateral cogwheel rigidity.
