AbstractThis article outlines a simple coding matrix for recording physical illness in a compact form suitable for computerization in research. It allows the recording of physical illness in a number of different ways: the particular organ system(s) involved; whether the illness is acute or chronic; the severity of illness; and the severity of interference with daily living. The application of the scheme is shown in a cohort of depressed elderly patients in hospital. Also included were three self‐assessment scales of physical illness and an OARS Physical Health Rating. It was concluded that the simplest and most parsimonious approach to assessing global physical illness for psychiatric research purposes on the elderly would be to (a) measure the number of body system affected by acute and chronic physical illness, and (b) ask each patient to assess his/her own physical health by one appropriate questio

AbstractThe associations between physical health problems and cognitive impairment, depression, generalized anxiety and phobic disorders are described in a random community sample of 890 elderly people. 28.4% of the total sample reported one or more serious physical health problems, and increased rates of health problems were associated with cognitive impairment, depression, generalized anxiety and agoraphobia. AbstractA subsample (N=204) of a consecutive series of 378 general practice patients aged 70+, previously given the Mental Function Test (MFT) and reported to show a prevalence of dementia of 0.8% (Jachucket al., 1986), was followed and survivors retested three years later. Dementia was categorized as definite, probable, possible or absent, based on AGECAT and on clinical diagnoses using ICD ‐10 criteria. The diagnoses were checked against patients' scores on the CAMCOG, MMSE, AMT, CAPE and Blessed Dementia Scale, and test‐retest differences on the MFT were noted. Patients who could not be interviewed were reassessed from their first MFT scores and the practice's research records. The MFT's performance and ability to predict death or dementia were examined. The prevalence of definite/probable dementia in the original sample is retrospectively reevaluated as about 11%. The rate in the general population aged 70+ is tentatively estimated after age adjustment to be approximately 9%, similar to that reported earlier (Kayet al., 1970). The age‐specific rates approximate to those derived from analysis of published data (Jormet al., 1987). It is concluded that the original estimate was too low. AbstractAll patients (N= 123) on the medical services of a chronic care hospital were approached to complete the Folstein Mini‐Mental State Exam and the Montogomery‐Asberg Depression Rating Scale. Of 63 patients completing the MADRS, 35% had scores in the depressed range. A comparison of MADRS scores for a depressed population of geriatric patients admitted to the psychiatric inpatient unit of the same institution showed that both depressed medical and depressed psychiatric patients were significantly different from the non‐depressed medical patients on all subtests of the MADRS except in the frequency of endorsement of ‘reported sadness’. There was no significant difference between the depressed medical patients and psychiatric inpatients. AbstractThirty‐four elderly residents of local authority homes (mean age 83) with depressive symptoms, some with a primary clinical diagnosis of dementia, others with a primary clinical diagnosis of depression, were further investigated and followed up in order to determine the validity of these diagnostic groupings. The two clinical diagnostic subgroups could not be distinguished by neuroradiological appearance on computed tomography and while those with dementia generally performed worse on psychometric testing, there was no statistically significant overall difference in the psychometric scores between the groups. Prognosis for those subjects with depressive symptoms was poor. There were no differences in outcome between those with a primary diagnosis of depression and those with a primary diagnosis of dementia, with only five subjects showing evidence of improvement in depressive symptomatology at one‐and two year follow‐up. AbstractMoclobemide was compared with mianserin in a double‐blind controlled study in elderly depressed patients. Sequential analysis of 11 pairs of patients found no significant difference between the drugs at the week 8 analysis. Unpaired comparison of the 26 patients completing four weeks' treatment also showed no significant difference between the two drugs. Both drugs significantly reduced Hamilton and Carroll depression rating scroes. A separate analysis of the 20 patients completing eight weeks' treatment showed that both drugs produced significant decreases in Hamilton and Carroll depression ratings with no significant difference between the two drugs for depression scores at either week 4 or 8.
