SYNOPSISCerebral blood flow (CBF) has been measured in 26 patients liable to cluster headaches. Of four patients studied during spontaneous attacks CBF fell in three and rose in one. Attacks could not be induced by nitroglycerine, alcohol or histamine except when patients were currently experiencing a bout of cluster headaches. The CBF rose during some induced attacks and fell during others. It is concluded that CBF changes do occur in cluster headache but the pattern is not consistent and does not at present suggest a mechanism for cluster headache.

SYNOPSISA survey is in progress of patients attending the Charing Cross Hospital Migraine Clinic in London. Information collected is stored for retrieval and analysis by computer. This paper reports some initial findings from patients seen during the first 212 years since the clinic opened.Patients registering with such a specialized clinic are not representative of the migraine population at large.Migraine sufferers present to the clinic at all ages from 6 to 70, but in three peaks, at 21–30, 41–45 and 51–55 years. Although onset of migraine tends to be earlier in males than in females, the latter are more likely to present during the first of these peaks.Migraine in younger clinic patients is predominantly classical in type. This predominance declines with increasing age at onset and may reverse when onset is delayed beyond 40.An inverse correlation exists in both sexes between age at onset of classical or common migraine and strength of family history. Females have a strong family history more commonly than males, except in those with onset in childhood. The mother is the more frequently affected parent, but involvement of the father may be associated more closely with early onset.Therefore, although migraine appears predominantly to be a disease of females with a female parental history, in clinic patients with early onset there is a relative preponderance of males and of male parental hi

SYNOPSISIn a generally healthy population, 75.8% complained of headache. Based on answers to a questionnaire, 18.9% were defined as suffering from migraine and 8.8% from tension headache. The prevalence of headache was more common among females and this was mainly due to the fact that migraine was more common in this sex. Abnormal visual acuity, abnormal (high or low) blood pressure, increased intraocular pressure, polycythemia and anemia occurred in the same frequencies in the headache and non headache groups (and in various subgroups). It is concluded that abnormalties in these physiological parameters cannot be a common cause of headache.

SYNOPSISBakal and Kaganov 1 recently reported data supporting a “psychobiological” approach to the study of headache pain. Their results suggested that individuals delineated by degree of self‐reported headache pain would differ on quantitative rather than qualitative headache characteristics. The present study was an attempt to systematically replicate and generalize the above findings. Seventy‐eight undergraduates and 25 chronic headache patients rated the extent to which common symptoms accompanied their “usual” headache on a five point scale. Subjects were divided into groups on the basis of: subjective ratings of headache problem, headache frequency, and headache type. Results were consistent with the psychobiological model. Individuals rating degree of headache problem as moderate or severe reported a significantly greater incidence of symptoms than non‐problem subjects. Headache frequency was not related to quantitative differences in headache symptomatology. Migraine and chronic muscle contraction (CMC) patients reported similar symptoms that differed significantly on quantitative (frequency) characteristics. The results replicate and generalize Bakal and Kaganov's findings and suggest that the model may help explain etiological factors in headache development. SYNOPSISAtenolol, 50 mg, b.i.d., and propranolol, 80 mg b.i.d., were given to 28 patients with a presumed diagnosis of migraine, in a placebo‐controlled double‐blind cross‐over study, where each period lasted 6 weeks. In the total material, atenolol was significantly better than placebo in reducing attacks, whereas propranolol showed no definite such effect. In seven patients, the number of headache days was remarkably high (average: 22 headache days in 42 days), which may indicate that they have been inappropriately included in the material or have recorded interparoxysmal headache in addition to attacks. SYNOPSISTwenty‐three patients were studied retrospectively for treatment received for post‐head injury. Over 90% of the patients treated made an excellent or good recovery.
