SYNOPSISThe migraine‐cluster headache syndrome is defined as headaches that are predominately of one type but with at least one major timing factor plus three lesser features of the other or five lesser features of the other (no more than one of the opposing features is a weak differential feature). The most important factors distinguishing migraine (M) from cluster headache (CH) are the timing of attacks (random occurrence versus clustering, frequency, duration, hour of onset) and the prodromal phenomena which occur with classical M, but are rarely seen with CH. Somewhat less significant differentiating characteristics are the associated symptoms of ipsilateral eye redness and tearing, nasal congestion and secretion, and partial Horner's syndrome which often accompany CH, in contrast to the symptoms of nausea and vomiting commonly associated with M. Also helpful in establishing the diagnosis are the quality, site and recurrent laterality of the pain, as well as its aggravating and ameliorating factors. Weak features in the differential diagnosis of M and CH are age of onset, sex, family history, and response to therapy.Patients with M or CH seen at the Headache Unit of Montefiore Medical Center during the past two years were evaluated for the occurrence of the migraine‐cluster headache syndrome. Nine (1.0%) of 923 patients with M had 4 or more features of CH and 5 (3.2%) of 154 patients with CH had 4 or 5 characteristics of M. SYNOPSISAdverse reactions have been previously reported when cerebral angiography is performed on patients with complicated migraine. We report a case of complicated migraine in which cerebral angiography produced come and prolonged suppression of EEG rhythms. Possible mechanisms are discussed.

SYNOPSISThe charts of 100 consecutive headache clinic patients were reviewed retrospectively to determine the frequency of prior dental treatment for TMJ disorders. Twenty had been treated, but only 4 satisfied our diagnostic criteria for the TMJ pain‐dysfunction syndrome. The other 16 satisfied conventional criteria for other headache diagnoses, and none of them had improved with dental treatment. SYNOPSISTo obtain a sample size sufficiently large to enable valid statistical analysis, patients with common or classical migraine treated in different centres with the calcium‐entry blocker flunarizine, 10 mg daily, were pooled into 2 sets. In a first group of 202 patients, flunarizine was shown to reduce the frequency of attacks in 83%. Improvement was more marked when the duration of the disease was shorter and was more rapid when the pretreatment frequency of attacks was high. Somnolence and weight gain, sometimes labeled increased appetite, were the main adverse reactions but rarely caused withdrawal of the drug. Weight gain was significantly correlated with the global result and occurred more in patients with a low pre‐treatment weight. SYNOPSISThe neck tongue syndrome is a rare clinical entity not well appreciated. In general, it has a good prognosis. We present two patients with this syndrome, both with novel additional symptoms. Existing clinicoanatomical and physiological correlates are discussed.

SYNOPSISMigraine patients, either during an attack or when pain‐free, have significantly higher platelet‐rich and platelet‐poor plasma methionine‐enkephalin levels than healthy race‐ and sex‐matched and age‐comparable controls. Although we did not observe differences in the platelet‐rich samples between the patients subgroups, platelet‐poor samples had higher peptide levels during a pain‐free period than the values obtained for the patients during a migraine episode. Similarly, platelet‐rich samples obtained from controls and patients during an attack had higher methionine‐enkephalin levels than their corresponding platelet‐poor plasma samples. SYNOPSISVarious biochemical determinations were performed on plasma and platelets of unmedicated common migrainous patients, either during crisis or in attack‐free intervals. The main change occurring during crisis appears to be a release of dense bodies and a‐granules, with a concomitant decrease of platelet serotonin uptake but without important modifications of the platelet metabolism of bioamines. SYNOPSISThe relationship between the major system of histocompatibility antigens and cluster headache has already been studied by our group, and decreased frequency of HLA‐B14 antigen shown in cluster headache patients in comparison to a control population.The therapy of cluster headache with lithium salts is already widely acclaimed; nevertheless there are patients who derive no benefit or must suspend treatment due to side‐effects. Thus we wanted to study the relationship between genetic markers of patients with the illness and the therapeutic efficacy of lithium salts.35 episodic cluster headaches have been studied; they had been already typed for the HLA antigens. The patients were treated with lithium carbonate for a period of 3 months. As parameters for evaluation of the efficacy of treatment, the headache index ratio, the wake‐sleep rhythm and the pupil diameter measurement were studied.Statistical evaluation was made and it was possible to separate two subgroups of patients: “responders” and “non responders” to lithium therapy. The phenotypical frequencies in the two subgroups was analyzed using the X2test.A higher frequency of HLA‐B18 antigen (23.8% vs 0%; p<0.0005; pc<0.06) and of HLA‐A9 antigen (42.9% vs 14.3%) showed in the “responders” group. SYNOPSISTwo young patients experienced transient and recurrent neurological deficits, associated with headache, without fever, signs of meningeal irritation or systemic disease.This situation in a young patient is usually a manifestation of migraine, but none of our patients had a history f vascular headache. Lumbar puncture was performed, and CSF pleocytosis found in both cases. These cases may represent a poorly recognized benign syndrome of transient neurological signs of headache and a sterile inflammatory reaction in the CSF.

SYNOPSISIn a double‐blind cross‐over study maprotiline was compared with placebo in the treatment of chronic tension headache in 30 women aged 16 to 52 years with a history of headache for 1.5 to 20 years. The treatment periods were 6 weeks long and the daily dose of maprotiline increased in two weeks from 25 mg to 75 mg nocte. The effects of the treatment ware measured by visual linear analogue scale, verbal rating scale, number of days without headache, number of days needing additional analgesics, and the patients' global impression of the treatment periods. In all these parameters maprotiline was superior to placebo and the difference was statistically highly significant. Beck Depression Inventory scores improved slightly during both placebo and maprotiline but the improvements wars without statistical significance. Patients not or only slightly depressed according to BDI responded better to maprotiline than those more severely depressed. Side‐effects ware frequent but mild and did not necessitate stopping the treatment.The results suggest that maprotiline is effective and useful in the treatment of chronic tension hea
