SYNOPSISThe study evaluates the contribution of activated parasympathetic and sensory nerve fibres in the sphenopalatine ganglion area, and of vasodilation in surrounding vessels in this narrow region (pterygopalatine fossa), to the pain and signs of autonomic dysfunction seen during attacks of cluster headache. Agents with anesthetic and vasoconstrictor effects were applied nasally to reach this area. It was found that the anesthetic effect is the most important, both regarding pain and autonomic symptoms. The effectiveness was also evaluated: this treatment is useful to stop attacks of cluster headache, but the beneficial effect is less than that reported in previous studies.

SYNOPSISThis study utilizes electronic thermography to investigate the response of 30 headache patients (migraine and mixed) and 20 non‐headache individuals to inhaling 100% oxygen at 10 liters per minute for 5 minutes. Observations were made prior to, immediately following, and at 3, 6 and 10 minutes post‐inhalation.Thermograms were inspected to determine the influence of hyperoxia on facial pattern temperatures across time. Chi‐square analysis determined that there was a significant difference (P<.01) in response between the control group and headache patients immediately following inhalation. While the temperature of each control subject decreased, 14 (46.67%) patients displayed a 0.5 to 1.0°C paradoxical rise. There was no significant difference in response between the types of headache. Statistically significant difference in response continued during the post‐inhalation phase; at 10 minutes, 6 (20.00%) patients continued to display temperatures above baseline, The response to hyperoxia did not differ if the patient was experiencing a headache, nor did oxygen significantly reduce pain. No difference in response was noted in those patients who were prescribed beta or calcium channel block‐ere. Hyperoxia increased migraine “cold patch” size. Regardless of temperature fluctuations, facial patterns remained stable across conditions. A blind calling of pre‐ and post‐inhalation thermograms was 93% accurate.The response of the medial canthi did not significantly differ between conditions. Hyperoxia generally increased size, temperature and symmetry. SYNOPSISThe diagnosis and treatment of facial pain is a multidisciplinary problem involving many areas of medicine and dentistry. Among the most common sources of this symptom are the dental structures.Facial pain of odontogenic and masticatory origin is often overlooked, perhaps because it is considered so obvious. The assumption that a patient enjoys good dental health may not always be true. Pain arising from not only the teeth and their supporting structures, but from the mandible, maxilla, the masticatory musculature and the temporomandibular joints, must also be considered during any examination of a facial pain patient.This paper considers pain of dental and craniomandibular origin. SYNOPSISA quantitative increase of leukocyte Leu7+ subset (largely represented by cells with natural killer activity, NK) was observed in the peripheral blood mononuclear cells of patients during the active period of cluster headache (CH). These patients are susceptible to herpes virus infections. In order to ascertain whether or not the quantitative changes in Leu7+ leukocyte subset would correspond to changes of the NK function, the activity of NK cells was investigated in CH patients. Eighteen patients with the episodic form of CH were studied during cluster periods and in painless periods. The control group consisted of 11 healthy volunteers. The results show that the NK activity of CH patients is significantly lower in comparison to that of control values. This impairment appears to be independent of the cluster period.Since β‐IFN has been shown to increase “in vitro” NK cell activity, studies were performed to investigate if β‐IFN could restore the limited NK activity found in CH patients. For this purpose the “in vitro” effect of overnight incubation with β‐IFN (1000 I.U./ml) was assessed on NK cell activity in CH patients and in the control group. SYNOPSISThe syndrome of greater occipital neuralgia (GON) is a cause of chronic unilateral or bilateral headaches. It occurs when the greater occipital nerve is compressed, irritated or inflamed. We describe a case of neurosyphilis in which GON was the presenting manifestation. We discuss this previously unreported early presenting feature of neurosyphilis and its implication.

SYNOPSISThis coded bibliography aims to include all behavioral headache research published to date. Four hundred and forty references have been found predominantly throughPsychological Abstractsand the journalHeadache. Each article has been coded according to seven variables which describe the subject sample, andthirteenvariables which describe the focus or topic of the study. Each reference preceded by the reference number, subject code, number of subjects in the study, and the topic code. Examples of subject codes are the types of headache the subjects were suffering from; and examples of topic codes are studies focussing on psychophysiological mechanisms, biofeedback training and personality. A table of reference numbers lists the articles pertaining to each topic starting with those including both tension headache and migraine subjects, followed by those including only tension headache subjects, then migraine only, and finally, ‘other’. SYNOPSISA 28 year old woman was admitted to the hospital with a 4 month history of headaches and blurred vision and a 2 week progressive loss of visual acuity. On examination she was found to have complete external ophthalmoplegia, florid papilledema and an otherwise normal neurologic examination. Cerebrospinal fluid pressure and protein supported the diagnosis of pseudotumor cerebri. A normal cerebral angiogram and “slit‐like” ventricles on brain computed tomography supported the diagnosis. SYNOPSISAssociations between HLA antigens and disease susceptibility have been investigated in several pathological conditions. Previous studies in selected migraine populations did not reveal any association with the HLA system. It is widely accepted that migraine is a disease with familial incidence. The genetic basis of migraine occurrence within 8 families with more than one member affected in two generations was studied. 60 individuals were studied; 33 of them were affected by migraine. HLA‐A,B,C,DR typing was performed in 41 individuals using the microlymphocytotoxicity technique of Terasaki.The studied pedigrees failed to identify a particular HLA antigen or HLA haplotype as a genetic marker of migraine. Among the total number of HLA typed individuals entering the study, Sib‐pair analysis was done in 20 pairs.
