SYNOPSISPain is a severe symptom in advanced oral cancer. Pain is often aggravated by the anxiety induced by visible oral cancer. Unless the primary and regional disease are controlled, nothing substantial in terms of pain relief can be achieved in advanced oral cancer by methods like radiotherapy, surgery or chemotherapy. However, additional treatments such as adequate medical and psychological management may result in considerable alleviation of pain. This article emphasizes the need for more organised education of health professionals, research and publications in cancer pain relief. It also underlines the importance of adequate medical and psychological management to alleviate pain from cancer.

SYNOPSISThe prophylactic effect of nimodipine in the treatment of migraine headache was assessed in a prospective, placebo‐controlled, double‐blind trial. Twenty‐six patients were studied during a 3‐month experimental period (placebo vs nimodipine) following a 1‐month period during which all patients received placebo. Results were tabulated using a “headache index”. A trend in favour of a beneficial nimodipine effect on the headache index was demonstrated which achieved statistical significance during one month of the follow‐up period. A similar trend with respect to the overall number of headaches was demonstrated and achieved statistical significance during two months of the follow‐up period. SYNOPSISThe tolerability and efficacy of naproxen sodium and of ergotamine tartrate plus caffeine (ergotamine) were compared in the treatment of acute migraine attacks and associated symptoms. In this multicenter, double‐blind, parallel study of up to six headaches over a 3‐month period, patients took naproxen sodium 825 mg, ergotamine 2 mg, or placebo at the time of the first symptom of an attack; 30 minutes later, if necessary, patients repeated naproxen sodium 275 mg, ergotamine 1 mg or placebo, as appropriate. Rescue medication was allowed 30 minutes following the second dose if needed. Active drugs provided notably better relief of head pain than did placebo; 1 hour following the first dose the difference between naproxen sodium and placebo was statistically significant. Naproxen sodium was as efficacious as ergotamine in the relief of migraine attacks and associated symptoms. Relief of vomiting, nausea, photophobia, and motor symptoms favored naproxen sodium over ergotamine; these differences were statistically significant for nausea and motor symptoms. Ergotamine‐treated patients reported more complaints and had more severe and longer‐lasting complaints than patients on the other two regimens. SYNOPSIS22 migraine patients, aged between 17 and 60 years (7 males and 15 females) were studied through the method of Visual Evoked Potentials (VEP). The control group consisted of an equal number of healthy subjects, comparable for age and sex. Correlations between the results obtained with VEP and those reported by traditional EEG methods were investigated. As found by other authors, VEP's did not point to any significant difference in common migraine as compared to a control group of healthy subject.Unlike other authors we found the presence of mildly abnormal EEG elements in only 19% of patients.

SYNOPSISIncreasing evidence of the similarity between muscle contraction headache (MCH) and migraine has been reported. However, there seem to be some problems and confusion about the notion that MCH and migraine are similar. For clarification of the relationship of MCH to migraine, we carried out a series of multivariate analysis studies. One hundred and eighty headache patients (including 60 MCH patients, 60 combined headache patients and 60 migraine patients) were investigated. Clinical data of these 180 patients were analyzed using Hayashi's Quantification analysis and cluster analysis. It was possible to determine an MCH factor and a migraine factor from headache symptoms and characteristics. A diffuse and sequential distribution of patients was observed in the histogram of quantified symptoms with the purpose of discriminating headache categories. Thus, most headache patients might have both the MCH factor and the migraine factor in various proportions. We propose a slight modification of the headache severity model and add some development of the concept that MCH and migraine are a continuum. We submit a new headache model.
