SYNOPSISChlorpromazine (CPZ), in doses ranging from 75–700 mg per day, gave full headache relief in twelve of thirteen (93%) of patients with the cluster headache syndrome. For nine, withdrawal of CPZ within two to three weeks was tolerated without recurrence of symptoms. After a period of extended remission, four of these found CPZ equally effective treatment for subsequent attacks. Three others received sustained relief from headache over six to eight months on maintenance CPZ at 75 mg per day although the cluster was not terminated. The apparent effectiveness of CPZ in this limited series is greater than that of other medications currently employed in the treatment of cluster headache

SYNOPSISRecent evidence has indicated activation of the serum complement system in some migraine sufferers. Using a new accurate method for the determination of C‐1 inhibitor in serum, we found an average of 17.33 mgdl (S.D. = 1.82) in 23 normal females. In 23 female migraineurs, the C‐1 average value was 17.87 mgdl (S.D. = 2.66). The value in 4 normal males was 18.82 mgdl (S.D. = 2.16) and in 4 male migraineurs 18.34 mgdl (S.D. = 3.36). SYNOPSISFinger and head temperature, frontalis electromyography (EMG), heart rate, and finger pulse amplitude (FPA) were measured from eight migraine headache patients both during and between migraine headaches. SYNOPSISMuscle contraction headaches have been demonstrated to significantly decrease following EMG biofeedback as well as general relaxation practice. This study contrasted the efficacy of a comprehensive biofeedback and stress management strategy and a self‐monitored home relaxation program in the treatment of 20 tension headache sufferers. The results suggested that while both treatment strategies successfully increased patients' ability to relax and reduce frontalis EMG levels, only the comprehensive biofeedback program was successful in reducing reports of headache pain and feelings of psychological distress. The findings support the utility of a headache treatment model beginning with simple relaxation and progressing on a case by case basis to more comprehensive tension and stress management strategies (like EMG biofeedback and cognitive stress coping techniques) only when the less costly relaxation proves ineffective in altering subjective feelings of pain or distres

SYNOPSISErgotamine alters the distribution of carotid blood flow over the capillaries and the arteriovenous anastomoses in favour of perfusion of the capillaries. This mode of action of ergotamine, which occurred independently of the concomitant reduction in carotid blood flow, indirectly supports the migraine shunt theory in which excessive opening of arteriovenous anastomoses is thought to be the primary hemodynamic derangement underlying the vascular headache of the migraine type.
