SYNOPSISThe cardiovascular reflex responses of 10 migraine patients were recorded during both migraine attacks andheadache‐free intervals. Ten healthy subjects of similar age and sex served as a control group.When the results of the measurements for migraine patients performing an isometric work test duringmigraine attacks and during headache‐free intervals were compared, during migraine attacks a statisticallysignificant smaller increase in diastolic B.P. was encountered than interictally (p<0.05).When the results for migraine patients during migraine attack, and controls, were compared, statisticallysignificant differences were encountered in the pulse rate variation in Valsalva manoeuvre, in an orthostatictest, and in the systolic and diastolic blood pressure reactions in an isometric work test. The mean Valsalvaratio and R‐R‐interval ratio during the orthostatic test were lower in migraine patients during migraine attackthan in controls, and the mean blood pressure rise in the isometric work test was also lower in the migrainepatients group during attacks.There were no statistically significant differences between the results of the migraine patients duringheadache‐free intervals and those of the control

SYNOPSISFive patients with basilar artery migraine (BAM) are described. One, while exhibiting electroencephalographic(EEG) interictal discharges and photo‐paroxysmal responses entirely suppressed by carbamazepine orvalproate, had no improvement of her attacks. Subsequent therapy with propanolol did improve the attacksdespite persistence of EEG abnormalities. A second patient had paroxysmal abnormalities and improved withisometheptene mucate. Three other patients had normal ictal and interictal EEGs, including after activationprocedures. A population of patients with basilar artery migraine exists with normal EEG recordings or withparoxysmal activity independent from that of migraine; these patients may respond to drugs other thananticonvulsant

SYNOPSISVisual evoked responses (VERs) and spectral analysis of background EEG were obtained from 20migraine patients and their age‐matched controls. Full‐ and half‐field reversing checkerboardstimulus presentations elicited no substantial differences between the subject groups for any of themajor component (N75, P100, N145) latencies or amplitudes. No VER differences were found forpatient subcategories based on side of headache, symptom history, or age. However, migrainepatients demonstrated slight but significantly higher mean frequency in their background EEG in the4–8 Hz frequencies in leads C 3 ‐P 3 and C 4 ‐P 4 , and a lower mean frequency in the 8–12 Hz band inleads F 3 ‐C 3 and F 4 ‐C 3 relative to controls. SYNOPSISWe examined the socio‐economic consequences of headache in a random sample of 514 working people living inthe Republic of San Marino. Our results show that, in the preceding year, 255 out of 514 (49.6%) suffered fromheadache; 31.9% (164/514) reported decreased efficiency as a consequence of headache; 9.3% (48/514) reportedthat head pain is always severe; 7.4% (38/514) were absent from work because of headache for a total of 338working days lost; 8.4% of all working days lost to illness were due to headache.We estimated that in the Republic of San Marino (21,792 inhabitants), headache causes a loss of at least 5,000working days every year. Since the cost of a working day in the Republic of San Marino is about 100,000 italianlire, the economic burden in this area amounts to 500 millions of lire in a year. SYNOPSISLeukotriene B4(LTB4) levels in plasma and its generationin vitrofrom isolated polymorphonuclearleukocytes (PMN) were measured in migraine and cluster headache patients during and between painattacks. The LTB4plasma levels in cluster headache patients during an attack were significantly higherthan in patients between attacks. There was a positive correlation between the time of sampling from thebeginning of attack and the LTB4level. The LTB4plasma levels in migraine patients during and betweenattacks did not differ significantly from control levels. The results suggest that LTB4appears rapidly in thecirculation at the beginning of a cluster attack and thereafter declines in amount. LTB4release from PMNwas induced by stimulation with the calcium ionophore A23187. The release of LTB4was significantlyhigher in migraine patients during attacks on stimulation with a submaximal dose of A23187 0.5 μM. LTB4release induced by A23187 in migraine patients during symptom‐free intervals and cluster headachepatients did not differ from healthy controls. LTB4release was not affected by 5HT and NA inconcentrations up to 10‐4M. The results suggest that LTB4is more easily generated in migraine patientsduring attacks. SYNOPSISIntensity of photophobia in dull and bright light was assessed in 39 migrainous patients and 15tension headache sufferers during headache, and in 20 nonheadache control subjects. In 24 patients,measurements were repeated during the headache‐free interval. Subjective estimates of glare andlight‐induced pain were greatest in patients with migraine at the time of examination, but ratings bytension headache sufferers also exceeded control ratings at most levels of illumination. In 19 of 25patients with unilateral headache, light‐induced pain was greater on the symptomatic side. During theheadache‐free interval, glare ratings in dull and bright light, and pain ratings in bright light, weregreater in headache‐prone patients than in nonheadache controls. Sensitivity to glare could thusform part of a general hyperexcitability of the special senses, while bright light appears to exacerbatepain resulting from activation of trigeminal pain pathways during

SYNOPSISThe effects of a psychophysiological treatment (relaxation training) and a psychological treatment(stress‐coping training) were tested in 29 patients suffering from migraine.There were significant pre‐post reductions in migraine variables and drug consumption. The effects didnot differ between the types of training, and remained stable for a medium‐term follow‐up of 8 months. Wethink, however, that stress‐coping training may in course of time be superior in maintaining training gains.Although this assumption cannot yet be tested for longer follow‐up periods, there was a non‐significanttrend for stress‐coping training to be more effective in migraine 8 months after training.There were different effects on social behavior: patients receiving relaxation training became moresocially withdrawn, while patients who underwent stress‐coping training increased their socialassertiveness. Furthermore, according to the participants, relaxation training had a stronger impact onmigraine, whereas stress‐coping training was more directed at migraine‐related stress. SYNOPSISPiribedil is a direct central dopamine receptor agonist drug, clinically useful in the diagnosis of migraine. It mayalso be used in the study of migraine pathogenesis, through evaluating dopaminergic “tone”.The observed effects of Piribedil on GH and PRL release, in this study, indicate the existence of dopaminereceptor hypersensitivity in migraine patients.
