SYNOPSISThere is often opposition to the establishment of special clinics. In spite of this, the last fifty years hasseen a gradual increase in the number of clinics serving specifically patients with headaches. While thereare wide variations, there can be little doubt that they provide the basic clinical material for research andwill, by the accumulating experience and expertise, best serve to help patients with headache disorders.

SYNOPSISAn epidemiologic study of headache in a rural general practice was carried out by delivering questionnaires to the houses of the target population and collecting them personally in two weeks. All persons between 16 and 65 years of age, 2,450 in all, were asked to participate in the survey; there was a response of 92%.Headache has been an underrated problem in Holland. The impact of headache on an individual patient can be assessed by the duration of the headache, the severity of the headache, and the extent to which usual daily work is interfered with. In this survey, 12% of the persons questioned had a headache at least once a week and in 4% the headache lasted longer than a day. However, only 15% of the people with headache visited a G.P. and less than 1% had specialist treatment. Women were more likely to seek medical assistance than men.

SYNOPSISThe dexamethasone suppression test was performed on eleven patients with cluster headache. Interest in such investigation of the hypothalamo‐pituitary‐adrenal axis in cluster headache arose from both a certain analogy with manic‐depressive disorders and recent data on abnormal patterns of cortisol secretion in cluster headache. SYNOPSISOur recent work focused on some immunological aspects of CH patients. We found a decrease in the expression of HLA‐B14 antigen on lymphocytes of these patients. In previous studies, we showed that testosterone plasma levels are low in these patients during the crises. It is possible to postulate a correlation between the variation of some hormonal parameter and changes of circulating leukocytes subsets (as in pregnancy and in the pre‐eclamptic syndrome).The following lymphocyte and monocyte subsets were studied using the monoclonal antibodies of Becton Dickinson series: Leu1 +, Leu2+, Leu3a+, Leu7+, Leu10+, ratio helper/ suppressor, LeuM3 +. We compared, using the ANOVA test, the values obtained during a phase of headache crisis versus those obtained out of crisis. SYNOPSISRecent studies have hypothesized that late‐onset immune response may be mediated through Immunoglobulin G (IgG) and Immunoglobulin A (IgA) in common migraine. In the present study, measurements of serum concentrations IgD, IgA, IgE, IgM and IgG were obtained on samples of healthy controls and common migraineurs. No significant differences in concentration of any of the isotypes were found between groups. Correlational analysis between isotypes within groups demonstrated that whereas the relationships between IgD and IgG, IgD and IgA, and IgG and IgE were positive in the control group, the correlations between these variables in the migraineurs were negative (inverse). Moreover, the application of semi‐partial correlational techniques to these relationships generated divergent models of regulatory process underlying isotype expression. In control subjects, IgD is involved in the expression of IgA and IgE, which are reciprocally related to each other and in turn affect the concentration of IgG. Common migraineurs were found to be characterized by a dysfunctional regulatory process as manifested in their pattern of isotype expression. IgD is involved in the independent expression of IgA, IgE or IgG, but IgA and IgE are not involved in the expression of IgG and there is no reciprocal relationship between IgA and

SYNOPSISWe evaluated 58 pediatric patients (39 females) suffering from various kinds of migraine in order to assess the prevalence of mitral valve prolapse, using echocardiography, and platelet function, using standard platelet aggregation tests. The patients were grouped as follows: 14 subjects with muscle contraction headache, 9 subjects with common migraine, 13 subjects with classic migraine, 22 subjects with complicated migraine. We examined as a control group 30 normal subjects matched for age and sex with the patients. Prevalence of MVP in subjects with complicated migraine (59%) was significantly higher as compared to the other groups of patients (r<0.005) and to the control group (r<0.0005). The prevalence of MVP in subjects with classic and common migraine (13.6%) was slightly, but not significantly higher than in the control group (6.7%). No significant differences in platelet aggregation tests were observed either between groups of migraine sufferers and controls, or between subjects with and without MVP. We suggest that the association between MVP and complicated migraine is not coincidence, and that follow‐up studies are needed to assess its clinical significanc
