SYNOPSISAutonomic nervous system functions were studied in 188 patients with migraine during headache free intervals by measuring cardiovascular reflex responses. Eighty‐five healthy subjects served as a control group.Pulse rate (R‐R‐ interval) variation in normal and deep breathing, during Valsalva manoeuvre and orthostatic test was diminished in migraine patients. Diastolic blood pressure rises in orthostatic test and isometric work test were lower than in the controls. The results of patients with classic and common migraine did not deviate from each other. The same was true for the results of the female and male patients.The results of the patients with frequent migraine attacks (>4/month) showed more severe disturbance than those with infrequent attacks. Disturbances gradually developed with age, being minimal or totally lacking in young migraine patients. SYNOPSISA double‐blind, randomized, placebo‐controlled trial was conducted to determine the onset of action of ibuprofen 400 mg in the treatment of muscle‐contraction headache. Because this represents a non‐prescription dosage of this analgesic, we evaluated subjects with a history of recurrent muscle‐con‐traction headaches that were amenable to over‐the‐counter analgesic therapy. Headache pain intensity was evaluated on a visual analog scale, and pain relief was measured on a categorical scale 15, 30, 45, 60, 90, and 120 minutes after a single 400‐mg dose of ibuprofen or placebo. Efficacy was detected within 15 minutes on the pain intensity rating scale (P<0.05) and within 30 minutes and at all subsequent time points on both the pain intensity and pain relief rating scales (P<0.001). SYNOPSISWe reviewed our experience with episodic cluster headache patients to evaluate both short and long term results of prophylactic treatment. 50 patients (46 M ‐ 4 F) with episodic cluster headache were studied. During a follow‐up period (mean duration 27.5 months, range 12–84) we studied: response to prophylactic treatment, relapses in responders, and response to the same prophylactic treatment when repeated in subsequent bouts. Drugs used were: Prednisone 50 mg/day; Nimodipine 120 mg/day; Lithium Carbonate 900 mg/day and Methysergide 6 mg/day. The patients were treated at the start of the cluster (within the first week of a bout); successful outcome was defined as the disappearance of attacks within seven days of treatment. Prednisone and Nimodipine gave positive results in over 50% of cases, Lithium Carbonate in 40% of cases, Methysergide in less than 30% of cases. Prednisone presented 33% of relapses; less relapses occurred with Lithium and Nimodipine.Treatment effectiveness tends to diminish in subsequent bouts irrespective of the drug

SYNOPSISNine patients with migraine (5 with migraineous stroke) were investigated for defects of mitochondrial energy metabolism. During effort, blood lactate rose significantly higher in migraineurs. Muscle biopsy showed ragged‐red fibres deficient in cytochrome‐c‐oxidase activity in one case. SYNOPSISPrecipitants of individual migraine attacks were studied in 23 migraineurs. Patients were aware of 91 precipitants (average 4 per patient); by direct questioning we could add a further 127 (average 5.5 per patient) provoking factors for migraine episodes.Avoiding these stimuli reduced attacks from 2 to 1g per month in 19 of the 23 patients without prophylactic drugs. Lessened severity or better analgesic control was noted by some. In 4 of the 23 patients the frequency of attacks remained unchanged.

SYNOPSISIt is now possible to obtain information about intracranial blood flow velocity by the use of transcranial doppler. We investigated transcranial doppler flow velocities in 42 cluster headache patients, and 41 controls. Interhemispheric asymmetry was significantly greater in the cluster headache group. Within the patient groups, velocities in the middle cerebral artery and posterior cerebral artery, were significantly faster on the headache side and slower in the anterior cerebral artery.Prophylactic medication seems to block these changes.
