SYNOPSISIn the light of therapeutic, biochemical, physiological, pharmacological and clinical evidences, we postulate that the headache syndrome is due to an autonomic nervous system (ANS) imbalance, triggered by many aetiological factors. Consequently, this syndrome is most often observed in those subjects whose ANS balance is precarious, i.e. neurotics and psychotics. Two main types of headache patients could be differentiated: (1) those showing predominance of the serotonergic (5HT) over the catecholaminergic (CA) system (depressive patients) and (2) those showing predominance of CA over 5HT system (manic or hypomanic patients).

SYNOPSISForty patients who presented to the Headache Research Foundation in Boston during the winter of 1978/1979 completed an interview, in dialogue with a computer, and then evaluated the program. Ninety‐five percent of the patients thought that the computer interview was easy to use, and 55 percent thought that the computer had obtained a complete history of their headaches. SYNOPSISThe value of the EEG for evaluation of patients with headache, and the interrelationship of migraine and epilepsy remain unsettled issues. Three patients with the syndrome of aphasia, headache, and left temporal spikes, all of whom were successfully treated with phenytoin, suggest a common pathophysiology. Since the symptoms in these patients lack some of the cardinal features of migraine, and yet fulfill definitions of epileptic disturbance, it is probably worthwhile to recognize this syndrome as an epileptic disturbance which appears responsive to phenytoin. These three cases also serve to emphasize the remaining value of the EEG as a diagnostic tool even in the age of radioisotope and computerized tomography scanners.

SYNOPSISThe efficacy of naproxen, a potent prostaglandin synthetase inhibitor, in migraine prophylaxis was assessed in a double‐blind placebo‐controlled cross‐over study in 28 migraine patients. After a drug‐free interval of at least 2 months, placebo and naproxen were given for 6 weeks each, with an interposed placebo week in between the two treatment periods. The dose of naproxen was 250 mg b.i.d. Naproxen seems to exert a moderate, but significant beneficial effect, with regard to headache days as well as headache indices. In a direct comparison, a significant number of patients preferred naproxen to placebo. Side effects were negligible. SYNOPSISMigraine frequently induces hypotension, which has a large postural component. Since pain and other symptoms of the migraine syndrome have been suggested to be central in nature, an involvement of the dopaminergic pathways within the central nervous system that mediate hypotensive responses, might be postulated in migraine patients. In the study, a test dose (2.5 mg) of bromocriptine, a brain dopamine agonist, induced an intense hypotensive reaction in migraineurs, but not in controls. During bromocriptine‐induced hypotension, serum dopamine β‐hydroxylase activity did not change. The fall of blood pressure, due to bromocriptine, in migraine patients could be considered as an indirect expression of brain dopamine receptor supersensitivity caused by a deficiency of dopamine and or endogenous morphine‐like substances. SYNOPSISWe have studied the effect of isometheptene, an indirectly acting sympathomimetic with a reported anti‐migraine action, on carotid blood flow and the distribution of carotid blood flow throughout the cephalic circulation of the cat, using 15μm‐microspheres. Microspheres measuring 15μm in diameter were used to separate capillary flow from flow through the arteriovenous anastomoses.
