SYNOPSISThe efficacy and safety of proquazone 150 mg and proquazone 75 mg was compared to placebo in a double‐blind, randomized, parallel, multi‐investigator study in the acute treatment of muscle contraction (tension) headache.Of the 122 patients entered into the study, 5 were subsequently discontinued, and 117 patients (39 in each of the three treatment groups) were included in the efficacy analyses.The effects of proquazone 150 mg on all of the symptoms associated with the muscle contraction (tension) headache symptom complex, namely pain, psychic tension and muscle stiffness of the head, neck and shoulders, were significantly superior to both proquazone 75 mg and placebo. These results were consistent using both a patient self‐rating form and a physician's global evaluation of patient response to study medication. For the pain parameters, proquazone 75 mg also showed significant superiority to placebo indicating a dose related clinical effect of proquazone. SYNOPSISTypical cluster headaches occurred in a patient who had had the ipsilateral eye removed eighteen years previously. This case, and similar ones in the literature, eliminate the eye as an important link in the pathogenesis of cluster headache.

SYNOPSISTwo hundred cases with recurrent idiopathic headache and 200 age‐ and sex‐matched controls without headaches were compared for the occurrence of medical problems and diagnoses according to their primary physicians. Hypertension, vertigo and dizziness, peptic ulcer, gastroesophageal reflux and depression and/or anxiety were among diagnoses seen more frequently in the headache cases, while nephrolithiasis, excessive ethanol use and abdominal pain in women were more common in the controls. There were no significant differences between the two groups in the diagnosis of cardiac disease, including mitral valve prolapse and ischemic heart disease, and occlusive cerebrovascular disease. SYNOPSISThe role of opiates in pain perception and neuroendocrine responses to environmental stimuli is well known, so that plasma and CSF endorphin levels may represent useful markers of endogenous analgesia and adaptive body mechanisms.The present study concerns various forms of headache:primary(Common Migraine: CM; Migraine with Interparoxysmal Headache: MIH; Cluster Headache: CH) andsymptomatic(Chronic Post Traumatic Headache: CPTH; Ischemic Cerebrovascular Headache: ICVH). A group of age‐matched healthy subjects was used as controls (C).The results show a significant decrease of plasma B‐EP levels only in MIH patients (p<0.001), while in the CSF a significant reduction of B‐EP is present in MIH (p<0.001) and CM (p<0.005) subgroups, coupled with an inverse relationship between peptide levels and headache severity scores. SYNOPSISInformation on the prevalence, characteristic features and possible causative factors of headache was collected from 1138 adults who participated in a multiphasic health survey in Milton in May 1981.Thirty‐nine percent of men and 60% of women experienced headaches in the year preceding the survey. Total prevalence of headache peaked in men 30–39 years and in women aged 20–29 years and thereafter decreased with increasing age.The most commonly reported positions for headache were at the forehead unilaterally and bilaterally. The most common precipitating factors were ‘too much’ alcohol (men only), tiredness, mental stress and eye strain. Chocolate was the food most commonly blamed for the headache.No association was found between headache frequency and blood pressure or smoking habit.
