ABSTRACTFifteen patients with prolonged benign exertional headaches are described. The headaches were mostly bilateral (nine), throbbing (eight), occurring during exertion and lasting an average of four hours. Their ages at the onset of the headache varied from eight to fifty‐four. In all of them, Neurological examination, neurological tests, including (CAT) computerized axial tomography, were normal. Indocin (indomethacin) completely or almost completely controlled the exertional headaches in thirteen out of fifteen patients (86%) in a period of one to four weeks. SYNOPSISEffort Headache may be produced by many sports activities and three cases are presented which occurred in runners. The clinical characteristics are similar to vascular headaches aggrevated by altitude, heat, high humidity and sexual intercourse. Although exertional headache should cause vigilance in evaluation, a benign type is usually seen in runners.

SYNOPSISSixteen physician‐investigators participated in a double‐blind, randomized clinical study comparing the efficacy of Micrainin (meprobamate‐acetylsalicylic acid) and aspirin (acetylsalicylic acid) in the treatment of muscle contraction (tension) headache. Each patient treated one episode of moderate‐to‐severe muscle contraction headache and scored the effects for head pain, activity impairment, tension (tense/uptight feeling), muscle stiffness and overall relief using a verbal rating scale (VRS) and a visual analogue (linear) scale (VAS). In general, overall agreement occurred between the two rating scales. Patient and physician global evaluations also concurred and supported the conclusion that Micrainin was significantly more effective than aspirin in relieving the symptom complex of muscle contraction headache. SYNOPSISThe relationship between migraine and epilepsy is reviewed. Recurrent paroxysmal episodes in children and their relationship to electroencephalographic abnormalities is discussed. Relevant literature concerning the 14 and 6/second positive spike pattern is also surveyed.The electroencephalographic data from a group of 116 pediatric headache patients was conpared to a group of 93 children previously diagnosed with seizure disorders. In the headache group, the 14 and 6/second positive spike abnormality was seen in 40.7% of the abnormal recordings, either by itself or with other abnormalities, and in 12.2% of all EEGs performed. This pattern was found in 6.5% of all abnormal EEGs and in 4.3% of all EEGs performed in the seizure group.It is hypothesized that the 14 and 6/second positive spike pattern is a marker for autonomic hyper‐reactivity and is most readily found in children under 16 years of age.The use of the electroencephalogram in the pediatric headache work‐up is emphasized to differentiate the “Seizure Equivalent” headache disorder from migraine. SYNOPSIS23 patients and 6 healthy controls were requested to participate in this study. Examination of skin samples consisted of light microscopic estimation as well as, transmission electron microscopic techniques (TEM) and immunohistochemical techniques. The total number of mast cells (MCs) was significantly higher in cluster headache (CH) patients than in controls, but differences between painful region and opposite temporal site were inconclusive. TEM search revealed presence of MCs around nerve fibres. There were no plasma cells containing polyclonal Ig, IgA, IgG and IgM according to horseradish peroxidase techniques. These observations suggest the role of MCs in CH pathogenesis, which support author's previous findings and results of others as well. SYNOPSISSuperficial temporal arterial (STA) dilation has long been associated with migraine pain, but it is still not clear if vasodilation alone is sufficient to produce pain. Extracranial‐intracranial bypass (EC‐IC) patients are known to exhibit STA dilation post‐operatively. A group of 56 E.C.‐I.C. patients was questioned about headache, during scheduled post‐operative examinations. Routine pre‐operative and post operative arteriograms of the STA were compared to verify vasodilation. Doppler sonograms were performed on eight of these patients to demonstrate systolic flow velocity changes and to evaluate velocity pulse curve profiles. Despite significant and sometimes marked arterial dilation, as well as pulsatile changes consistent with migraine, none of the subjects complained of post‐operative headache. SYNOPSISAlthough numerous pharmacological and behavioral treatments of muscle‐contraction headache have been introduced, the psychophysiological mechanisms underlying this pain disorder have not been identified. This paper reviews and critiques laboratory and treatment studies which add to our understanding of the psychophysiological factors involved in muscle‐contraction headache. The results of a number of studies are consistent with a hypothesized muscle‐contraction or vasomotor etiology. However, other studies have reported results inconsistent with these traditional etiological hypotheses. Interpretation of results is hindered by numerous methodological problems, particularly inconsistent and unreliable diagnostic procedures and insufficient controls for alternative etiological explanations. Although causal inferences are difficult to draw, it is evident that significant individual differences exist in the degree to which muscle‐contraction headaches are associated with elevated cephalic muscle tension and that muscle tension alone is insufficient to account for the occurrence of muscle‐contraction headache. SYNOPSISBasilar migraine may produce temporary change in brainstem function resulting in impairment of activity of medullary autonomic nuclei. A case of basilar migraine with transient cardiac arrhythmia is described and it is suggested that abnormalities in cardiac rhythm may be detected by cardiographic monitoring in some cases of basilar migraine. This report emphasizes the occasional occurrence of serious complications during a migraine attack.
