SYNOPSISIn order to determine the generality of the Kudrow‐Sutkus (1979) MMPI classification system for diagnosing headache type, a cross‐validation analysis was performed on 99 headache subjects. Results indicated a high level of support for Kudrow‐Sutkus' Group A (migraine and cluster) classification criteria (65.4% and 75.0% correct assignments for female and male subjects, respectively) but a low level of support for Kudrow‐Sutkus' Group B (combined and chronic scalp muscle tension) classification criteria (5.9% and 25.2% correct assignments for female and male subjects, respectively). SYNOPSISA clinical pain syndrome similar to "carotidynia" developed in a patient several years after undergoing carotid endarterectomy. The pain was reversed by superior laryngeal nerve block, followed by superior larnygeal neurectomy. A diagnosis of superior laryngeal neuralgia was suggested by several characteristic features: (1) pain along the anterior cervical triangle, with extension to the ipsilateral ear and eye, (2) hoarseness, and (3) paralysis of the ipsilateral cricothyroid muscle on laryngoscopy. Carotidynia usually refers to neck pain arising from the carotid artery in the neck and is often viewed as a migraine variant. Our observations suggest that carotidynia may not be a migraine variant and that "carotidynia" may not be an accurate term for all pains in the anterior cervical triangle. We suggest that evaluation of neck pain include speech pathology and otolaryngologic consultations (including laryngoscopy) if any voice disorder is reported or noted. Since the superior laryngeal nerve is the neural structure most contiguous to the bifurcation of the carotid artery, the superior laryngeal nerve may have become entrapped in a fibrotic process that developed after carotid endarterectomy. Such pain may be a rare complication of carotid endarterectomy. When other causes have been excluded and pain continues, a superior laryngeal nerve block should be considered.

SYNOPSISThe effect of ergotamine tartrate was compared with that of the antiemetic agent metoclopramide and with those of two combinations in a double‐blind trial of 24 adult female patients with migraine. The following combinations of the drugs were used in oral administration in a total of 176 acute migraine attacks: (a) Ergotamine 1 mg, (b) Metoclopramide 20 mg, (c) Ergotamine 1 mg + metoclopramide 20 mg, (d) Ergotamine 2 mg + metoclopramide 20 mg. The duration of attacks was significantly shorter on both of the combinations compared with the single drugs. The intensity of the pain was somewhat weaker and the appearance of nausea and vomiting somewhat but not significantly less during the combination treatments. In their overall opinion the patients favored the 2 mg + 20 mg combination significantly more than the others. Both ergotamine and metoclopramide are efficient in acute migraine attacks. Their combination seems to enhance the therapeutic response in some respect

SYNOPSISThe McGilI‐Melzack Pain Questionnaire (MMPQ), comprised primarily of adjectives descriptive of pain, was administered to 100 patients seeking treatment for headache. Diagnostic classification of patients into migraine or muscle contraction headache groups was conducted by a screening neurologist using information other than pain description. Reliability determinations were made following independent diagnosis by two other neurologists using a headache pain history and symptom form (HPHSF) devoid of pain adjectives. Comparison of 30 subjects HPHSF's revealed that the screening neurologist's diagnosis and that of two other neurologists, (t = .87,p<.01) and (t = .48,p<.05), indicate high agreement between three physicians in headache diagnosis devoid of pain description. Results show that migraine headache patients report significantly more affective words (t99= 3.89,p<.001) than do muscle contraction headache sufferers; however, no significant differences existed between the groups' use of sensory or evaluative works. Migraine sufferers, while not reporting their headache as more severe “generally,” did report more intense pain when recalling headache at its “worst” (t99= 2.69,p<.01) and at its “least” (t99= 1.74,p<.05) compared to the muscle contraction headache 'group. Discriminant analyses were conducted on one‐half of the sample to determine diagnostic group membership on the basis of pain description alone. Findings revealed that group membership could be predicted at a 90% rate (x21.45= 33.06,p<.001). A cross validation on the second half of the sample confirmed these findings (x21.48= 13.08,p<.05) suggesting that the MMPQ is of value in headache diagnosis. SYNOPSISThe hypothesis that oral tyramine causes migraine headache in certain patients was proposed by Hanington in 1967. In all, there are 11 published reports that experimentally test the hypothesis. Six of these studies provide support for the hypothesis whereas the results of four studies are clearly not supportive. All of the supporting evidence was generated in one laboratory (Hanington). In an attempt to evaluate this conflicting evidence, a comparison was made of outcome measures, subject selection procedures, and various methodological differences. There were some anomalous findings, most notably that there were marked differences in "placebo" headaches elicited by lactose capsules from study to study. Further, methodological differences among the studies preclude direct comparisons of the results. Taking these differences into account, the tyramine hypothesis appears to have some validity. The implications of these findings were discussed.

SYNOPSISIn a case of common migraine and in two cases of cluster headache we monitored the arterial pressure and the heart rate by means of polygraphic recordings before, during and after headache attacks.During migraine attacks the arterial pressure did not show significant changes; the heart rate increased. During cluster attacks the arterial pressure increased with frequent hypertensive peaks; the heart rate on average slowed down and showed marked irregularities.

SYNOPSISTwenty chronic post‐traumatic headache subjects (16 men and 4 women) were considered, together with age and sex matched normal controls. Pituitary function was challenged by means of luteinising hormone releasing hormone (100 mg,iv), thyrotropin (TSH) releasing hormone (200 mg,iv) and regular insulin (.1 U/kg,iv). The responses of gonadotropins, TSH, somatotropin (STH) and prolactin (PRL) were studied in the group as a whole and in clinical subgroups. No differences were found in the baseline levels of the hormones. No significant changes were observed in the responses of gonadotropins and STH. A decreased response was found for TSH and, in the female series, an enhanced response of PRL was observed. Patients complaining for a decreased libido showed a larger PRL response. The smaller response of TSH exhibits a straight inverse relationship to the clinical severity. The periodicity of PRL secretion studied in male patients with a harmonic analysis according to the Fourier's theorem showed the preservation of the physiological sleep‐entrained night surge. The per cent incidence of the harmonics with a period shorter than 24 hours decreases according to the clinical severity, at variance to the finding in normal controls. The only persisting harmonic is the one with a 24 hour period. In the absence of major post‐traumatic endocrine syndromes, post‐traumatic headache subjects show changes of the neuroendocrine regulation of pituitary secretion.
