ABSTRACT.Consecutive survivors of a myocardial infarction from the Southern Hospital, below 70 years of age, were randomized into a Control group (n=276) and a Treatment group (n=279). The latter was openly prescribed the combination of clofibrate and nicotinic acid for serum lipid lowering. Each patient should remain in the study for 5 years and be seen regularly every 4 months at a special IHD outpatient clinic within the hospital. The concentration of serum cholesterol and triglyceride was lowered by 13% and 19%, respectively, in the Treatment group compared to the Control group. Total mortality was 82 cases in the Control group and 61 in the Treatment group, a 26% reduction (p<0.05). For patients above 60 years of age in the Treatment group the reduction in mortality was 28% (p<0.05). IHD mortality was reduced by 36% (p1.5 mmol/l (n=216). Secondly, it was most pronounced in the 44% of the treated patients who had a lowering of the serum triglyceride concentration by 30% or more, and in this subgroup the reduction of IHD mortality was 60% (p<0.01). For serum cholesterol there were no such relations. The difference between serum triglycerides and cholesterol concerning these relations to the treatment outcome may be due to the fact that hypertriglyceridaemia was the most common hyperlipidaemia among our patients, occurring in 50%, while hypercholesterolaemia only occurred in 13 %. Caution should be exercised in the interpretation of the results as the trial was not blind. However, the fact that the decrease in IHD deaths was directly related to the degree of serum triglyceride lowering indicates that it was the drug effect on serum lipids that was reponsible for the beneficial effect of the treatment.

ABSTRACT.The concentration of lipids, lipoproteins and apolipoprotein A‐I and B was measured in the plasma of 33 patients, enrolled in a double‐blind, controlled trial of alprenolol in myocardial infarction, after one year on the study medication and again after 6 months off the medication. Sixteen patients received 200 mg alprenolol twice daily and 17 received placebo. There were no statistically significant differences between the parameters in the two groups after one year on medication. However, when medication was stopped, the ratio of apolipoprotein B to apolipoprotein A‐I fell by 9% in the alprenolol group and increased by 2% in the placebo group. This difference was statistically significant. Our results suggest that alprenolol, a β‐blocker with weak intrinsic sympathomimetic effect, has slight effects on plasma lipoproteins. ABSTRACT.The analgesic effect of ketobemidone hydrochloride + the spasmolytic component A29 (Ketogan®) and morphine hydrochloride was compared double‐blindly in patients with suspect acute myocardial infarction. The test drugs were administered i.v. in an initial dose of 0.5 ml (2.5 mg Ketogan, 5 mg morphine) followed, if necessary, by additional injections of 0.25 ml. Altogether, 309 patients participated in the trial. The total comsumption of the test drugs showed that 5 mg Ketogan was equipotent with 10 mg of morphine. Treatment with Ketogan resulted in a significantly higher proportion of patients who were completely free of pain 15 and 30 min after the initial injection: 16% and 15% more, respectively, compared to morphine. Within 2 hours after the initial injection, approximately 15% of the patients in both treatment groups had reported nausea and about 7 % had vomited (patients who vomited or were nauseated before treatment were not included in this analysis). The frequency of other side‐effects was low, with no differences between the two treatment groups. Morphine caused a greater reduction of the systolic blood pressure than Ketogan. ABSTRACT.We studied the influence of β‐adrenoceptor blockade on heart rate responses to facial immersion in ice‐cooled water after acute myocardial infarction. Forty patients with acute myocardial infarction were randomized to the double‐blind therapy with metoprolol 200 mg daily or matching placebo. Therapy was started as soon as possible after admission to hospital and continued for 15 days. After this initial period all patients were treated with β‐blockade during the subsequent 6 months. Repeated investigations of heart rate responses to facial immersion in ice‐cooled water and 24‐hour ECG‐recordings were performed on day 15 (while patients were on study therapy), after 6 weeks, 3 and 6 months. Throughout the 6‐month period, facial immersion elicited significant heart rate reductions in both treatment groups. No differences were found between the two groups or between different examinations during follow‐up. There was no correlation between heart rate response to facial immersion and frequency of ventricular arrhythmia on a 24‐hour monitoring. In conclusion, acute administration of metoprolol in myocardial infarction does not influence the bradycardial response to facial immersion either in the early postinfarction phase or during 6 months' follow‐up. ABSTRACT.Plasma concentrations of 5000 daltons (5 kDa) immunoreactive gastric inhibitory polypeptide (IR‐GIP) were measured before and up to 16 hours after the start of low‐dose insulin treatment in newly diagnosed ketotic type I (insulin‐dependent) diabetics. Nine patients were non‐fasting. Before insulin treatment mean IR‐GIP was 31 ±6 pmol/l (range 9–65 pmol/l). Four patients had IR‐GIP concentrations in the normal fasting range (10–25 pmol/l), and nine patients had concentrations below 35 pmol/l. The remaining patients had IR‐GIP concentrations in the normal postprandial range. A meal eaten after the start of insulin treatment caused an increase in IR‐GIP in all patients. All patients had β‐cell function as estimated by plasma C‐peptide. Individual changes in C‐peptide were significantly correlated to changes in blood glucose both after the meal (r=0.80,p<0.01) and during insulin treatment (r=0.85±0.04). No correlation could be found between IR‐GIP and blood glucose, C‐peptide or insulin concentrations. Newly diagnosed ketotic type I diabetics have IR‐GIP concentrations within the normal postprandial level. Hypoinsulinaemia, hyperglycaemia, and hyperketonaemia do not by themselves increase 5 kDa IR‐GIP

ABSTRACT.Questionnaire responses from 120 men and 337 women over the age of 50 years were studied to determine the prevalence of back pain among the elderly. In order to gain a rough indication of the back pain among elderly women which might be due to osteoporosis, the prevalence was compared in the two sexes. The prevalence of back pain without radiation to the legs and concomitant morbidity was found to be similar among men and women up to the 70–79‐year age‐group. After this age the prevalence was higher in women. Those with exceptional loss of body height or kyphosis had a high prevalence of back pain, while those who had sustained previous hip or radius fractures did not. There was increasing prevalence of back pain among women with increasing number of previous fractures. ABSTRACT.The influence of chronic iron deficiency anaemia on myoglobin content, maximal enzyme activities and capillarization in the human skeletal muscle was investigated. Muscle samples from musculus vastus lateralis were screened in an Indonesian population. The causes of iron deficiency were chronic intestinal bleeding or repeated pregnancy combined with low iron intake. The maximal activities of iron‐dependent and non‐iron‐dependent glycolytic and oxidative enzymes as well as myoglobin showed similar values in the iron‐deficient group and the matched control group. The activities of the oxidative enzymes in both the iron‐deficient group and the controls were lower, however, compared even to untrained Swedish subjects. The capillary density was essentially within a normal range in both groups. It is concluded that chronic iron deficiency anaemia of a moderate or severe degree, with Hb concentrations of about 80–100 g.1‐1, does not cause an impaired biochemical function of the human sk

ABSTRACT.To investigate the pattern of change in endogenous overnight creatinine clearance during the third trimester, 12 healthy women with uncomplicated pregnancies were examined three times a week. Urine was collected overnight from 22.00 to 08.00 hours and analysed for creatinine. Serum was sampled in the morning and analysed for creatinine, β2‐microglobulin, progesterone and estradiol. The general trend of creatinine clearance was parabolic with a declining level during the last month before term. A sinusoid pattern with minimum values around the time when the women would have had menstruation had they not become pregnant was superimposed on the parabolic trend. A mathematical model (parabolas overlaid with a cosine curve) was constructed and fitted to the data. The cyclical pattern was significant. Serum creatinine showed a pattern with increasing values during the last 4–6 weeks before term and cyclical changes which were also significant. In the individual case the monthly and preterm clearance decrement sometimes was over 50%. Monthly and preterm decreases in creatinine clearance may be quite normal and serial measurements of creatinine clearance are therefore necessary to determine if declining values indicate pathological falls in the glomerular filtration
