ABSTRACT.The prevalence of hypertension in the age group 40–69 in the County of Skaraborg, Sweden (265 000 inhabitants) was estimated using diagnostic criteria commonly accepted in Sweden in recent years. The prevalence was higher than expected (16%) and most of the patients (80%) were previously identified and undergoing treatment. The quantitative aspect of hypertension was even more impressive in the ages above 69 years. Eighteen per cent of the men and 33 % of the women aged 75 years were on antihypertensive medication. An unexpectedly large proportion of the population had a borderline pressure requiring further follow‐up (16%). ABSTRACT.The feasibility of a nurse‐based care program for hypertension has been evaluated. The program was established in one half of the County of Skaraborg, the other half being selected as control area. Record forms from the diagnostic work‐up and annual check‐ups of all 3 240 patients registered during five years were reviewed. Compliance with diagnostic procedures and treatment was excellent. At the end of the five‐year trial period, 7% of the population studied (40–69 years old) were registered at special outpatient hypertension clinics. Random samples were drawn of patients treated at these clinics and patients taken care of in the control area. These patients were interviewed concerning their general opinion about the care, their opinion about nurses as the main deliverers of care, the continuity of care and needs for information. ABSTRACT.Aiming at a better control of hypertension, a medical care program including nurse‐run outpatient hypertension clinics was implemented in one half of the County of Skaraborg (265 000 inhabitants), the other half forming a control area. The program was evaluated during a 5‐year trial. Among patients registered and followed up for a minimum of one year (n= 2806), systolic and diastolic blood pressure decreased significantly, regardless of the patients' status of treatment prior to admission to the clinics. A baseline and a terminal population study revealed an increasing effectiveness in the treatment of hypertension in the study area. A consistent difference (mean 8/5 mmHg) was found in average blood pressure between patients treated at the hypertension clinics and patients treated in regular care. ABSTRACT.We have studied the effect of β‐adrenoceptor blockade on the pyrexial response to acute myocardial infarction (AMI). Temperatures of 22 patients treated with timolol, 20 mg daily, in the acute phase of AMI were compared to temperatures of 22 patients not receiving timolol treatment. Fever response after AMI was significantly reduced in the timolol‐treated patients, maximal and mean temperature in the febrile period being lower and the febrile period being shorter. Reduction of fever in AMI may be of importance as a higher body temperature increases the risk of developing heart failure and arrhythmias. ABSTRACT.One‐hundred‐and‐thirteen patients with endocarditis and valvular insufficiency were studied retrospectively with special regard to indications for operation and the optimum time for cardiac valve surgery. Thirty patients (group I) had acute, 63 (group II) subacute and 20 (group III) prosthetic valve endocarditis. Group I: Eleven patients underwent surgery in the acute stage, 8 while bacteremic; 5 of the latter died perioperatively. Of the 19 patients treated medically, 16 died. Group II: All patients underwent operation in a bacteria‐free state. The mortality was 5%. Group III: Eight patients had early (<60 days postoperatively) and 12 late endocarditis. Total mortality was 40% (71 % early and 25% late mortality). Ten patients underwent reoperation, with a mortality of 20%, compared with 60% in the medically treated group. ABSTRACT.The usefulness and feasibility of recording atrial wall motion by M‐mode echocardiography guided by two‐dimensional examination was evaluated in three groups of consecutive patients'. 7 with undefined tachyarrhythmias, 25 in sinus rhythm, and 20 with atrial flutter or fibrillation. Atrial systole was recorded in the left and right atrium in 58 and 98% of the patients, respectively (p<0.05). Six of the patients with undefined tachyarrhythmias exhibited electrocardiographic atrioventricular dissociation revealed by preceding echocardiography in all. The precise timing of left and right atrial systole could be recorded in patients in sinus rhythm; right atrial contraction preceded left atrial systole by 42±31 msec (mean ± SD). Among patients with atrial flutter or fibrillation, one case of dissimilar atrial rhythms was revealed by echocardiography. ABSTRACT.α1‐Antitrypsin (AAT) deficiency of phenotype PiZ and idiopathic hemochromatosis (IH) predispose to the development of cirrhosis and liver cell carcinoma. Several reports have suggested an association between these two inborn errors. To elucidate this question we used a monoclonal antibody against the PiZ gentic variant of AAT to analyze and compare the PiZ gene frequency in an area (county of Jämtland in Central Sweden) with a high, and another area (the city of Malmö in Southern Sweden) with a low IH prevalence. The PiZ gene frequencies did not differ between the areas. We also analyzed sera from 27 unrelated adult males with hemochromatosis diagnosed in the high IH area for the presence of the PiZ gene product but none was a carrier of the PiZ allele. ABSTRACT.One hundred and eight patients with phlebographically verified deep venous thrombosis were treated with streptokinase. Total or partial thrombolysis was phlebographically demonstrated in 60 patients (55.6%). Three patients died during treatment, all from pulmonary embolism. Six patients developed clinical signs suggestive of pulmonary embolism. In 16 patients (14.8%), major bleeding complicated the treatment. One patient had anaphylactic shock, while various allergic reactions were recorded in 22. Streptokinase therapy in the routine management of deep venous thrombosis carries an acceptable efficacy and safety similar to what has been achieved under research conditions.

ABSTRACT.Serum amylase, isoamylase and lipase were determined in 17 patients with pancreatic or biliary diseases before and after endoscopic retrograde pancreatography (ERP). Within ½‐2 hours after cannulation of the pancreatic duct, serum lipase was increased to approximately 4 times the upper reference level and normalized almost completely at 24 hours. A much smaller increase was found in amylase and isoamylase. The elevation in enzyme activities was less in patients with abnormal than with normal ERP.
