ABSTRACTA cohort of 1 019 male and 1 232 female hypertensives, aged 25–59 years, based on a random population sample, was followed for five years during a community‐based cardiovascular prevention programme. A small mean reduction in serum cholesterol level was found. The observed changes in casual serum cholesterol values were partly due to the regression to the mean. The reductions were most marked in elderly people and in those with high baseline serum cholesterol values. The partial regressions of the cholesterol change were computed in subgroups by age, sex and baseline serum cholesterol level. Changes in weight in men and in age in women were the strongest independent predictors of the change in serum cholesterol. Changes in dietary fat intake were also associated with the change in serum cholesterol. Only a small part of the total variation in the change in serum cholesterol was explained by the regression models. ABSTRACTSerum angiotensin I converting enzyme (ACE) was measured in 10 patients with Graves' disease and 2 with thyroiditis during different stages of the diseases. The effect of thyroxine on serum ACE levels was also recorded in 12 patients with thyroid cancer, who were on thyroxine suppression. Serum ACE levels correlated positively with clinically assessed thyroid function and peripheral thyroid hormone levels, especially during hyper‐or hypofunction. ACE was measured both with an enzyme kinetic and a new, quantitative inhibitor binding assay. The methods gave similar results, which indicates that ACE increments during thyroid hyperfunction were quantitative, and not a result of increased enzyme activity. Serum ACE increments associated with high lysozyme concentrations are signs of immunologic activation or proliferation of monocytic cells. In this study there was no correlation between the two enzymes, which may indicate either increased synthesis or possibly shedding of ACE from endothelial cells or delayed metabolic clearance of this enzyme. ABSTRACTSystolic time intervals (STI) and radionuclide ventriculography (RNV) were used in the assessment of cardiac performance in 22 patients with hyperthyroidism before and after antithyroid treatment. STI as well as RNV showed enhanced myocardial contractility which was normalized after antithyroid treatment. Beta‐blocker treatment did not influence the hypercontractility in hyperthyroidism. This implies that β‐blockers alone cannot be regarded as satisfactory long‐term treatment and that mechanisms other than increased sympathoadrenal drive are involved in the hypercontractility in hyperthyroidism. ABSTRACTElectrocardiograms, serum electrolytes, plasma concentrations of pre‐albumin and retinol‐binding globulin, and dietary intakes were analyzed in 22 women during weight loss after gastroplasty surgery for morbid obesity. QT interval corrected for heart rate (QTc) was prolonged (>0.44 sec) in 32% (95% confidence limits 14–55%) on one or more occasions. No clinical or electrocardiographic complications were seen. Occurrence of QTcprolongation was significantly (p<0.05) associated with protein intake below recommendation and with low plasma pre‐albumin concentrations. QTcprolongation was not associated with mineral intake and occurred in spite of normal serum levels of calcium (uncorrected and albumin‐corrected), magnesium, potassium and sodium. ABSTRACTTo assess the acute cardiovascular effects of cimetidine—a widely used histamine‐H(2)‐receptor antagonist—cuff arterial blood pressure, M‐mode echocardiogram of the left ventricle and systolic time intervals were recorded in 10 healthy volunteers before and after 200 mg of cimetidine or isotonic saline given intravenously in a double‐blind crossover manner. Neither echocardiograms nor systolic time intervals revealed any significant effects of cimetidine on the left ventricular performance. However, cimetidine decreased slightly the systolic arterial pressure. The maximal effect of cimetidine (7±1 mmHg, mean ± SEM) differed significantly from that of saline (3±1 mmHg,p<0.02). When compared with the pre‐injection level, the calculated total peripheral resistance decreased significantly after cimetidine (p<0.01), whereas saline induced no such change. We conclude that a bolus injection of cimetidine does not impair cardiac performance but may induce transient hypotension due to reduction of the total peripheral resistance. ABSTRACTThe effect of cimetidine (1 g daily) and placebo was studied in a controlled clinical trial comprising 50 patients with non‐ulcer dyspepsia in whom an organic abnormality responsible for the dyspeptic symptoms was not disclosed by a standardized and extensive examination programme. Reduction of symptoms occurred in 13 (54%) out of 24 patients treated with cimetidine and in 16 (62%) out of 26 treated with placebo. The difference was insignificant, as were the alterations in the individual dyspeptic symptoms between the groups. Only 6 patients (25%) on cimetidine and 8 (31%) on placebo treatment had a total relief of symptoms. Of these, all cimetidine‐treated patients remained free from symptoms during the successive 3‐month observation period, while the dyspeptic symptoms relapsed in 3 (38%) placebo‐treated patients. Subsequent resumption of placebo treatment reduced the symptoms in all 3 patients, but only one became free from symptoms. ABSTRACTFourteen patients with pericardial effusion of various origins underwent pericardiocentesis on 16 occasions. Incipient cardiac tamponade was present on 19 occasions in 12 patients. Pericardiocentesis was carried out bedside under sterile conditions by a trained cardiologist. In case of impaired central circulation, drainage was sufficient to stabilize the clinical condition haemodynamically, and no adverse events occurred during pericardiocentesis. Fenestration of the parietal pericardium was necessary in 5 patients because of rapid regeneration of the fluid and in one patient due to pyopericardium with cardiac constriction. Pericardiocentesis is recommended in incipient cardiac tamponade. In the hands of experienced cardiologists the drainage procedure can be carried out safely without sophisticated equipment.

ABSTRACTA double‐blind trial with the β1‐selective blocker metoprolol in suspected acute myocardial infarction and during 3 months' follow‐up included 1 395 patients, aged 40–74 years, 698 on metoprolol and 697 on placebo. In order to further evaluate the tolerability to β‐blockade in the elderly, the total series was divided into 2 groups according to median age (61 years) and into quartiles, the lowest quartile (40–57 years) being compared with the highest (67–74 years). The decrease in heart rate and systolic blood pressure after intravenous metoprolol in the acute phase was similar in the elderly and the younger patients. Hypotension was observed more often in the metoprolol‐treated than in the placebo‐treated younger patients, while no difference was observed in the elderly. Bradycardia was observed more often in the motoprolol group in both age groups, while there was no difference regarding the incidence of congestive heart failure in either the younger or in the elderly patients. The effect on mortality, serious ventricular arrhythmias and chest pain seemed to be similar in different age groups.
