ABSTRACTA health survey of middle‐aged men was carried out in 1970‐73 in the municipality of Uppsala. Subjects with hypertension, hyperlipidaemia, reduced glucose tolerance, and smokers were invited to join various therapy groups. By 1980 this multifactorial intervention programme had thus been running for 10 years. This report describes the results of a follow‐up undertaken to evaluate the efficacy of the programme. The annual rate of fatal myocardial infarction (MI) was lower among the participants (n=2322) in the health examination as well as among participants and non‐participants (n=446) combined than among the male Swedish population of the same age (162 and 187 compared with 296 per 100 000 men, respectively). The annual rate of non‐fatal MI among participants and non‐participants combined was 295 per 100 000 men, which is lower than in other Swedish cities. In the hypertensive group (n=126), six men had fatal and seven non‐fatal MI. These 13 men had higher blood pressures (BPs) from the start than the other hypertensives. In addition, their BP reduction was smaller than in a control group randomly selected among the hypertensive subjects. In the hyperlipidaemic treatment group (n=363) there were eight fatal and 10 non‐fatal MIs. Nine of these events occurred in individuals who had dropped out from therapy. ABSTRACTBone metabolism was estimated by serum alkaline phosphatase (index of bone formation) and fasting urinary excretions of calcium and hydroxyproline (indices of bone resorption) in a group of early postmenopausal women and a group of 70‐year‐old women, during 12 months' treatment with 1,25‐dihydroxycholecalciferol (1,25(OH)2D3), and compared to oestrogen/gestagen treatment or placebo treatment. The groups treated with 1,25(OH)2D3did not show any change in bone metabolism, neither in bone resorption nor in bone formation, during the treatment period when compared to the placebo group, whereas treatment with female hormones decreased both bone resorption and bone form

ABSTRACTMyoglobin has been measured in sera from 305 consecutive patients with suspected acute myocardial infarction (AMI) to study the clinical value in relation to other diagnostic methods. On admission the frequency of false negative (i.e. the diagnostic sensitivity) myoglobin values was 28% in the AMI group as compared with 60% for serum creatine kinase (CK) and 46% for serum aspartate aminotransferase (ASAT). Four hours after admission the corresponding figures were 2, 31 and 29%. This makes the diagnostic sensitivity of the myoglobin test 0.98, which is significantly higher (p<0.001) than that of the two enzyme tests. The predictive value of a negative myoglobin test was 0.97 and also significantly higher (p<0.001 andp<0.01) than for CK and ASAT. S‐myoglobin was further related to the number of complications and the prognosis of the patients, and high levels appeared to be an unfavourable sign, particularly in combination with an anterior wall infarct. This study has demonstrated and confirmed the superior diagnostic sensitivity of myoglobin determination in early AMI. ABSTRACTThe results of treatment with continuous ambulatory peritoneal dialysis (CAPD) of 24 unselected patients with end‐stage diabetic nephropathy are reported. The total treatment time was 346 months. Six patients died, eight received a kidney allograft, two returned to CAPD after graft failure, six were transferred to other forms of dialysis and one to another centre. Five patients remained on CAPD. Good control of symptoms, biochemical values and hypertension was achieved. Intraperitoneal administration of insulin resulted in very good metabolic control of diabetes. ABSTRACTSera from 8 patients with IgA glomerulonephritis and from 6 patients with Schönlein‐Henoch nephritis were investigated by the Clq‐binding assay (Clq‐BA) test and by sucrose gradient centrifugation followed by sensitive solid phase radioimmunoassay determinations of IgA, IgG and IgM in each fraction obtained. Abnormal sedimentation profiles were found in sera from 3 of 8 patients with IgA nephritis and in 3 of 6 patients with Schönlein‐Henoch nephritis. The abnormal fraction consisted of low molecular weight IgA in 2 patients, IgG in 1 patient and IgM in 4 patients. The absolute level of polymeric IgA was increased in 3 of 8 patients with IgA nephritis, but the relative distribution was normal. ABSTRACTThe effects of glipizide on the absorption of glucose andd‐xylose were studied in six type II diabetics on diet treatment alone. Glipizide was given intravenously (12 μg/kg at 0 min) or orally (5 mg at ‐30 min). Oral glucose (15 g) and xylose (25 g) loads were given at zero time. Glipizide stimulated insulin secretion and reduced glucose and xylose levels significantly with both routes of administration. The suppression of xylose levels lasted longer after oral than after intravenous administration of the drug. ABSTRACTSedimentable cells from human adipose tissue of different origins were cultured under conditions when cell multiplication was prevented but lipid filling optimized (suspension culture). In this way an estimation was obtained of the number of in vivo determined adipose precursor cells (preadipocytes) which were not filled with lipid. Under these conditions no cells from adult, non‐obese subjects developed to adipocytes. Cells developing to adipocytes constituted less than 0.02% and 1% of adipocytes in tissues from obese adults and from children, respectively. ABSTRACTForty patients, 32 women and 8 men, with hyperthyroidism were investigated concerning serum lipoprotein lipid and apolipoprotein concentrations before, during and after treatment with carbimazole to euthyroidism. During the hyperthyroid condition the patients had significantly lower serum concentrations of low density lipoprotein (LDL) and high density lipoprotein (HDL) cholesterol. The serum lipoprotein triglyceride concentrations were within the normal ranges. During treatment to euthyroidism the low serum concentrations of LDL and HDL cholesterol increased to normal values. LDL and HDL concentrations increased in the female patient group by 46±12 and 25±9% and in the male patient group by 50±12 and 19±11%, respectively. ABSTRACTSerum from a woman with a history of Hashimoto's thyroiditis, who had given birth to two children with congenital hypothyroidism, contained potent TSH blocking activity. Immunoglobulin preparation from this serum abolished completely TSH‐stimulated cAMP production in human thyroid membranes. The blocking activity was associated with the IgG fraction absorbed to and eluted from a Protein A column. The stimulation of adenylate cyclase by a preparation of thyroid‐stimulating antibodies from a patient with Graves' disease was also inhibited by the antibodies. In contrast, no effect was observed upon fluoride‐stimulated cAMP production. The data indicate that the antibody activity was directed against the TSH receptor. Immunoglobulin preparations from 22 other patients with Hashimoto's thyroiditis and 16 patients with subacute thyroiditis were examined for the existence of TSH receptor blocking antibodies. A blocking activity was found in two of the 22 Hashimoto patients. No such activity was found in the patients with subacute thyroiditis.
