ABSTRACT.The tenth Norwegian patient with familial LCAT deficiency is reported. Her family lives in the same area as the three previously reported Norwegian families. ABSTRACT.Five cases with candidiasis‐endocrinopathy syndrome are presented. They are clustered in three distantly related families with two pairs of affected sibs and one single case. All cases have idiopathic hypoparathyroidism, four have idiopathic Addison's disease while chronic mucocutaneous candidiasis has appeared in three cases. The mode of presentation argues for an autosomal recessive inheritance. Varying degrees of defective cellular mediated immunity were found. HLA typings revealed no evidence for HLA linkage. ABSTRACT.Urinary cyclic AMP (cAMP) and phosphate were measured before and after calcium infusion in 12 patients with operatively verified primary hyperparathyroidism (PHP) and in 12 healthy persons. In normal subjects infusion of calcium caused a reduction in urinary cAMP directly correlated to the preinfusion values and inversely correlated to the serum calcium concentration determined as albumin‐corrected serum calcium. In normal subjects with high normal albumin‐corrected serum calcium the infusion of calcium caused no or only a small depression in the urinary excretion of cAMP. Changes in phosphate excretion were not correlated to the calcium concentration. Four of the 12 hyperparathyroid patients showed normal relative suppression in urinary cAMP after the infusion of calcium, and 5 had normal suppression of phosphate excretion. ABSTRACT.A number of 2203 middle‐aged Icelandic men (aged 34–61 years) participated in a health survey in Reykjavik in 1967‐68. Blood glucose in the fasting state and during an oral glucose tolerance test (OGTT) showed a unimodal distribution with a slight skewness towards higher values. A positive age gradient was established, which showed an increment of about 7 mg/dl per decade in capillary blood sugar values at 1 ½ hours after a 50 g oral glucose load. The age gradient was mostly independent of age‐related changes in body weight. A 7 ½‐year follow‐up of the eligible participants (response rate>80%) showed an overall 1% incidence of metabolic deterioration to “overt diabetes” during this period. Those with positive screening tests (50 g OGTT) at baseline were at greater risk of developing diabetes, but the incidence of deterioration in this group was low, about 1% per year. The progression to diabetes was significantly related to body weight at baseline, suggesting that weight reduction might be beneficial in individuals showing impaired glucose tolerance at medical examination. ABSTRACT.To study the effect of verapamil on the glucose tolerance and insulin response to oral glucose, 6 healthy subjects and 15 patients with non‐insulin‐dependent diabetes mellitus (NIDDM) were investigated. In the healthy individuals, 3 tests were performed on separate days: 1) A standardized oral glucose load, 2) an identical glucose load during i.v. infusion of verapamil, and 3) a third oral glucose load after one week of oral verapamil treatment. No significant differences were found when the insulin and glucose responses with and without verapamil were compared. The patients with NIDDM were divided into three groups. Two tests were performed on separate days in each patient: group 1, a standardized oral glucose load and an identical glucose load during i.v. verapamil infusion; group 2, a standardized oral glucose load and a similar glucose load after one week of oral verapamil treatment; group 3, the protocol of group 2 was followed but oral placebo was given instead of verapamil. In groups 1 and 2, verapamil improved the tolerance for oral glucose, whereas the insulin response remained unaffected. In group 3, no significant differences were found in the insulin and glucose responses to the two oral glucose loads. These findings thus imply that verapamil improves the tolerance for oral glucose regardless of the route of verapamil administration in patients with NIDDM. ABSTRACT.A number of 738 patients with diabetes mellitus (DM) and 371 matched controls answered a questionnaire constructed to detect rheumatic hand symptoms. The DM patients showed significantly more palmar thickening, indicating Dupuytren's contracture and flexor tenosynovitis, and a significantly lower frequency of neurological sensations. There was no statistically significant difference in the estimated frequency of rheumatoid arthritis between the total population and our DM population.

ABSTRACT.In a population study in Göteborg, Sweden, comprising women in the age strata 44, 52, 56, 60 and 66 years, serum thyrotrophin (S‐TSH) was determined in 1283 women representative of the general population of women of these ages. No women with previously unknown hypothyroidism were found at the clinical examination; 47 (3.7%) were on thyroxine treatment, 9 (0.7%) because of spontaneous hypothyroidism.Sixteenwomen (1.2%) had markedly elevated S‐TSH concentration (≥14 mU/l). They were studied further by determination of thyroid hormones and circulating antithyroid microsomal and thyroglobulin antibodies, and those with goiter were subjected to fine‐needle biopsy of the thyroid gland. They were furthermore followed for four years.Foureuthyroid women were given thyroxine because of goiter and abnormal laboratory test results. Ofnineindividuals with high S‐TSH concentration as well as high titers of thyroid antibodies and/or biopsy evidence of autoimmune thyroiditis, seven became hypothyroid within the four years of follow‐up. The other two women had consistently elevated basal S‐TSH concentration and exaggerated S‐TSH response after thyroliberin administration. We conclude that the majority of individuals without previously recognized thyroid disease who have S‐TSH concentration above 14 mU/l with the assay used will require thyroxine therapy within a few years. Inonewoman, however, with multinodular colloid goiter and high titer of antimicrosomal antibodies, the basal S‐TSH concentration decreased during follow‐up to a high normal value; the S‐TSH response to thyroliberin was high borderline. ABSTRACT.The prognosis of 64 patients with atrioventricular (AV) block among 128 consecutive patients, 65 males and 63 females, treated with permanent pacemakers, was compared with that of 56 patients with sick sinus syndrome (SSS) during a mean observation period of 32.5 months. A poor survival rate was found in patients with known coronary heart disease (CHD) compared to those with isolated conduction defect (ICD). Among patients with CHD, a higher mortality was found in patients with AV block than in those with SSS (58 vs. 24%). Likewise, among patients with ICD the mortality in AV block was higher than in SSS (31 vs. 15%). Regression analysis showed a mortality excess in AV block vs. SSS (p=0.05) and a poor prognosis related to advanced age (p=0.02), males vs. females (p=0.04). The mortality excess in patients with CHD vs. ICD was significant (p=0.01). It is concluded that the increased mortality rate in patients with AV block is related to widespread myocardial damage, this being especially pronounced in patients with CHD.

ABSTRACT.Seven hundred and ninety‐five consecutive patients with the diagnosis of angina pectoris were studied by coronary angiography and followed for 2–7 years. The prognosis is greatly determined by the extent of coronary artery involvement. Concomitant mitral insufficiency or ventricular aneurysm influence the prognosis adversely. There was no significant difference in prognosis between men and women.
