AbstractHome apnea monitoring has been used increasingly for infants with or at high risk for apnea. This study examined mood states, perceived family functioning, and perceived support systems of mothers with infants on home apnea monitors from the first week on the monitor through three months after discharge. It also compared these factors in mothers receiving care through an established home monitor program or through their own physicians. Greatest discrepancies in all three areas of study occurred during the first week at home. During this time the group receiving care through private physicians reported higher discrepancy in professional support than that in the home monitoring program. No other significant differences were found between the two groups across time periods. Community health nurses have an important role in assessing and providing counseling concerning the mood disturbance and changes in family functioning that may take place when infants are receiving this therapy, particularly in the initial time at home.

AbstractThe task of providing infants adequate amounts of nutritious milk, in spite of mothers' ability to produce breast milk, has never been easy. In America cow's milk became the food of choice when mothers ceased to breast‐feed their infants. In the mid‐1800s advances in bacteriology and chemistry led to a beginning understanding of the role of microbes in food and the development of commercial proprietary formulas. Many mothers discontinued breast‐feeding at an early age and relied on these products for their children. With the growth of cities, dairy farms were pushed many miles from the consumers of milk. The distance the new uncooled milk had to be transported, especially during the hot summer months, increased the chances of milk‐borne diseases infecting infants. To combat the high rates of infant gastroenteritis and death, welfare reformers and health professionals banded together to improve the milk supply through pasteurization, certification, and safe distribution; and to educate the public, especially mothers, on ways to feed and keep their infants healthy. Through their close contact with mothers in their homes, nurses taught these women how to prepare, store, and feed infants’milk. When their babies survived the summer, mothers developed a sense of trust in the nurse and the things she taught about ways to protect their infants and the entire family

AbstractKalamazoo, Michigan, public schools have not had nurses employed by the board of education for over a decade when nurses were cut from the school budget as an austerity measure. Approximately 26,000 school‐age children live in Kalamazoo County, including a large inner‐city, low‐income population, many of whom have meager access to health care. Two part‐time public health nurses provide consultation and limited direct services. Nazareth College's commitment to community service has resulted in the placement of nursing students in elementary school settings. The nursing faculty orients the students and takes responsibility for their experience. These students have planned and implemented health‐promotion projects that have been well received. Suicide prevention, disaster response, nutrition, and personal hygiene are examples of topics. The challenge for school nursing is as great today as it was in 1902 when Lillian Wald first identified a need for school nurses. AbstractThis mental health training program was designed to increase primary care nurses' knowledge and skills about depression. The program emphasized criteria for assessing depression, presented psychopharmacologic and psychotherapeutic content, discussed care coordination among several agencies and providers, and addressed referral resources. Cultural and developmental issues were highlighted. The 237 participants had significant knowledge gains after the program on comparison of pretest and posttest measures. Client record audit found significant increases in the assessment of and intervention with depression. The training program successfully increased primary care nurses' abilities to use knowledge about depression in clinical practice.

AbstractIn the past 20 to 30 years the number of sheltered care facilities in the United States has risen dramatically. Serving what many consider to be marginal populations, they are often poorly regulated or unregulated, with little or no attention to the health of residents. A retrospective record review of 647 clients residing in boarding homes, rooming houses, a homeless shelter, and residential facilities in Monmouth County, New Jersey, suggested that whereas some differences exist among facilities in terms of clients’physical and psychosocial health problems, these differences may not be meaningful. Since New Jersey regulations require some minimal health supervision to be provided by residential facilities but not by the others, these results suggest that regulations of the other facilities should be revised to reflect better the needs and problems of the populations they serv

AbstractAlthough the quality circle (QC) process has been used in health care, there is a conspicuous gap in the literature about its use in community health nursing. The purpose of this service/education project was to implement QCs in the public health nursing sector throughout a southern central state. The major objective was to provide QC training to approximately 250 supervisors and staff nurses so that this participative group problem‐solving approach might be used as a systematic method of dealing with concerns related to quality of care. Evaluation tools, such as the Science Research Associates' attitude scale and the quality management maturity index, were used to determine whether or not the implementation of the QC program influenced the level of morale and quality management maturity. AbstractA group of North Carolina public health nursing leaders initiated a two‐phase process of providing a continuing education module on nursing diagnosis for all public health nurses in the state. In the first phase the authors developed the module to teach the skill of nursing diagnosis and evaluation tools, and statewide workshops were held to teach public health nurse administrators and nurse consultants how to use it with their staffs. Results of the evaluation revealed a significant increase in cognitive learning and a more positive attitude overall toward nursing diagnosis among 148 participants. Written comments revealed two major concerns for using nursing diagnoses: the need for additional practice, and charting issues. Both were addressed in this project.
