AbstractThis article analyzes the effects of federal fiscal policy in the Reagan administration on primary care programs. Within the context of the national economy and federal policy priorities, states responded to budget cuts and block grants with fiscal and administrative restrictions that fell disproportionately on disadvantaged groups. A wide array of primary care services became less available and accessible, thereby limiting their cost‐savings potential and their effectiveness in supporting people's health. These program deficits, together with lowered living standards among large segments of the population, resulted in rising rates of infant mortality in many states. A perspective that can interrelate federal and state policy actions, and broad public policies with traditional health policies, will improve public health nurses' understanding of policy‐making and policy impacts, and thereby can guide renewed efforts toward health improvem

AbstractA group of clinical nurse specialists in primary care examined unemployment through the frameworks of stress/coping and relative deprivation theories. The moderating effect of social support was also considered. The impact of unemployment on health was elicited from unemployed clients and records kept by the nurses providing care. Nursing diagnoses were found to fall into three major categories: subsistence problems, increased psychosocial stress, and lack of necessary health care. Nursing interventions included exploration and constructive mobilization of emotional responses and assistance with problem solving, including support of existing strengths and identification of alternative behaviors and resources. The need for advocacy on behalf of clients emerged as a continuing concern.

AbstractTheoretically, the focus of community health nursing is the community; however, the nature of the practice has not been well documented. The purpose of this study was to develop a community‐focused model by identifying appropriate behaviors and activities for staff‐level community health nurses. A list of suggested behaviors and activities was compiled by means of brainstorming sessions and sent to a sample of community health nursing administrators, educators, and staff nurses in Michigan. Responses were analyzed using nonmetric multidimensional scaling techniques to uncover and define the primary underlying dimensions of perceived community‐focused practice of community health nursing. Three specific regions were identified and labeled: (1) client‐oriented services; (2) aggregate needs identification; and (3) aggregate planning and intervention. AbstractThis study examined the effects of group education classes on self‐care agency (SCA) of individuals with chronic obstructive pulmonary disease (COPD) and assessed the four independent variables of presence of a significant other, severity of disease, social status, and type of lung disease, and the persons' relationships to SCA and to the change in SCA after education. A sample of 25 individuals with COPD who attended group education classes offered by the American Lung Association of Connecticut served as subjects. Prior to and at the conclusion of the series of classes, they were given questionnaires eliciting perceptions of SCA and biographic data. From the questionnaires, two SCA scores were calculated. A pairedttest demonstrated that these individuals had a significantly greater SCA at the conclusion of the classes than at the beginning. Stepwise multiple regression analysis of the biographic variables identified above and SCA revealed a significant negative correlation between chronic bronchitis and/or emphysema and SCA. There were no correlations between biographic variables and the change in SCA scores. AbstractThis study examined the relationship of perceived parental instrumentality and perceived parental expressiveness to future time perspective in tenth‐grade adolescents. Of the originally selected 270 students from an urban high school, 141 from twoparent families comprised the final sample. They responded to the modified Cornell Parent Behavior Description Questionnaire(s), the Heimberg Future Time Perspective Inventory, and the Child Personal Inventory Sheet. The data obtained on the modified Cornell Parent Behavior Description Questionnaire(s) were factor analyzed to determine the maternal and paternal scales of instrumentality and expressiveness. Using the Pearson product‐moment correlation coefficient, three of the four hypotheses tested were supported (p<0.05). Each hypothesis was further analyzed according to the sex of the adolescent and a different pattern of findings emerged.
