AbstractA project to define and test population‐management roles in nursing was implemented in a large HMO. Three patient populations were selected: diabetics, patients with multiple sclerosis, and pregnant adolescents. Expert nurse clinicians in each of the three areas piloted the role for six months, including establishing a system of care coordination for the populations. In all three pilots, patient care outcomes were improved. AbstractThis article seeks to describe the current efforts of public health nurses involved in family violence prevention while on home visits, participating in community groups, working on special projects, and developing innovative policies. Despite a dearth of outcomes research and data regarding public health nursing practice in this area, both strengths and weaknesses in public health nursing practice are evident. AbstractThe need for specialized HIV education for nurses working in rural areas will increase as the incidence of HIV infection increases in rural areas. Public health nurses provide a viable alternative to providers in acute care facilities and will continue to be the primary care providers for persons with HIV/AIDS in rural areas. While approaches to HIV/AIDS education should include clinical treatment as the core, clinical knowledge alone will not promote the development of caring communities. The Rural‐Based Nurse Model provides a comprehensive curriculum that addresses the many complex issues associated with the care of persons with HIV/AIDS. Additionally, participants are linked with care providers who serve clients across the continuum of HIV disease. Through meaningful educational opportunities and provider networking, this program has the potential for improving the quality of care in rural areas for persons with HIV/AIDS. HIV/AIDS education programs will require individualized community strategies that consider existing resources and barriers. AbstractThis study examined the ability of two specific measures of stressors associated with smoking cessation to act as predictors of progress through the stages of smoking cessation as described by Prochaska and DiClemente (1983). Specifically, a 19‐item scale measuring barriers to smoking cessation and a 14‐item scale measuring smoking cessation self‐efficacy were completed by 127 smokers and self‐quitters at three time points over a six‐month period. Subjects who made progress through the stages of smoking cessation had lower barriers scores initially and had significant changes in their self‐efficacy and barriers scores over the six‐month period. In contrast, non‐progressors had no significant change in either their self‐efficacy or barriers scores over the time period. AbstractThe role of public health in a reformed health care system is currently a matter of great debate at the national and state levels. Many public health nursing leaders have expressed concern about changes in public health policy and funding sources that have led to the near demise of the generalist public health nurse. Generalist public health nurses may be an endangered species; however, the role is still in existence in rural Alaska. Will they be one more example of how “what we learn from history is that we don't learn from history?” Taking the discrete area of the Yukon Kuskokwim Delta of Alaska and studying the forces of geography, politics, economics, social factors and health issues, this article provides an historic account of the development of a unique health care delivery system and the role of the public health nursing structure within it, from earliest known records to present day. AbstractCommunity participation is an important feature of community health promotion. One form is participatory action research. This article describes the process and outcomes of participatory action research conducted by a Healthy City in rural Indiana and public health nurse faculty. The community was integrally involved in the entire process, including framing of research questions, construction and distribution of survey tools, analysis of findings, and taking action on the results. Combining local insights of the community with technical assistance from public health nursing yielded multiple benefits. The role of participatory action research is appropriate for public health nurses because it is consistent with the goals and characteristics of the profession.

AbstractChildren with nonorganic failure to thrive (NOFTT) comprise a population at risk for small stature, poor growth, slower development, and lower intellectual outcomes. These children are often seen in the Women, Infants, and Children Supplemental Nutrition Program (WIC) and child health clinics and in caseloads of high‐risk families. Public health nurses may not be sure how to intervene in the problem of NOFTT because of its multifactorial etiology. A model of nursing care that addresses the many factors that affect the development of NOFTT can enable the public health nurse to appropriately care for the child with NOFTT. The Eco‐Epi model, a combination of human ecology and epidemiology, is a conceptual model that can provide a framework for the public health nurse to plan interventions. Even though children with NOFTT are the target population for this model, it can be applied to other public health nursing populations at risk for multifactorial problems. In the Eco‐Epi model, the epidemiological concepts of agent (food), host (child), and environment (home) are examined in the context of the microsystem (parent‐child interaction, daily activities of the family), the mesosystem (interactions between different environments), and the exosystem (the child's community). AbstractThe prevalence of chronic leg ulcers has been investigated in older people living in the community. This condition has a low prevalence, but major health costs are associated with frequent home nursing visits, community nursing services being the main providers of care. An estimate was made of the prevalence of chronic leg ulcers in a cross‐sectional random sample of noninstitutionalized older people living in a major Australian city (N= 1,050). The prevalence estimate is compatible with estimates of 1 % in the over‐60 age group found in Swedish and Northern American studies. Age‐adjusted gender ratios demonstrate slightly higher prevalence in females than males (1.25:1). Implications for community nursing services for primary, secondary, and tertiary prevention are outlined and include education programs for seniors dispelling myths that leg ulcers are a natural part of aging, screening for venous and arterial peripheral vascular disease in the older community, collaborative nurse and podiatrist foot care clinics, and the location of leg ulcer clinics in geriatric day care ce
