AbstractThe purpose of this study was to identify research questions and priorities in public health nursing based on the perceived needs of practicing public health nurses, and to assess whether the respondents believed that the profession should provide leadership in the study of the identified questions. Using a modified Delphi survey. 76 research priorities were ranked and produced three factors or categories of questions during the two rounds: outcomes in maternal‐child and family planning, outcomes in home health services, and public health nurse recruitment, retention, job satisfaction, and image. Forty‐seven (62%) of the 76 priorities were deemed appropriate for nursing to assume leadership in seeking answers. AbstractNurses as professionals and as individual consumers are affected by health policy and bear a responsibility for participating in the health policy arena. Nursing's growing recognition of the importance of public policy issues is increasingly being translated into action by various groups within the profession. Persuading student nurses of the importance of health policy provides a special challenge to faculty. A clinical practicum was designed that requires senior baccalaureate students in various community settings to identify, analyze, and challenge health policy issues. The course design, integration into the curriculum, and evaluation can be models for faculty considering the challenge of stimulating student interest in health policy issues.

AbstractFactors that influenced use of physician and nurse help were explored for 49 mothers of healthy, term‐born infants for two study periods in the first three months. These factors were concepts pertinent to mothers' internal working models of help use, their resources, perceived problem‐solving competence, and clinical evaluation of maternal competence in use of help. Variables contributing to the frequency with which help was used and to the ratio of that help to other communal help sources differed for nurse and physician help and for the time period. For physicians, the number of infant caretaking issues a mother identified contributed to the frequency of help use in both study periods. The problem‐solving help nurses gave emerged as a critical help in the second study period. The ratio of help from nurses to that from other sources was higher when mothers identified reassurance from the nurse as a critical help and, in addition, as the infant grew older, when the nurse's evaluation of the mother's use of help was more positive. In contrast, throughout the first three months, the more infant care experience a mother had, the higher the proportion of help from physicians relative to communal sources of help. AbstractThis prospective study sought to identify sociodemographic and psychosocial risk factors for unintentional infant injuries that occurred in or around the home. It also examined whether social support modified the relationship between high maternal stress and infant injury. The subjects were 367 mothers who were interviewed six to eight weeks after their newborn infants were discharged from the hospital, and approximately one year later, when 132 infants (36%) were reported as injured (burned, poisoned, serious fall, airway problem). Logistic regression analyses established that family conflict was the most significant predictor of unintentional infant injury, followed by fewer than two siblings living in the home at the first interview, and maternal unemployment. In addition, among mothers with high stress, the use of social support resulted in fewer unintentional infant injuries.

AbstractAssessing urban Native American (NA) parenting remains a challenge for public health nurses. This study explored the effectiveness of using the NCATS and HOME instruments with urban NA mothers and their children. Scores for 63 sets of mothers and their children age 3 months to 3 years on the home observation for measurement of the environment (HOME) and nursing child assessment teaching scale (NCATS) instruments were compared with norms for those instruments. Scores were lower than the norms on the play and involvement subscales of the HOME, and higher than the norms on the total parent, clarity of cues, response to parent, and total child subscales of the NCATS. Analysis of responses to specific items indicated that these mothers were unlikely to structure and intervene in children's activities and likely to use nonverbal parenting techniques. Findings supported the usefulness of these instruments with NA parents when accompanied by discussion of findings with them, and nurse awareness of common family structures and traditional values.

AbstractWe explored the influence of nursing dependency over and above sociodemographic factors and medical conditions on the use of home care nursing resources by patients 65 years of age and older. Use measures were number of visits made by home care nurses, and length of nursing stay. Nurses' reports, direct observation of nursing care provided, patient interviews, and record abstraction were used to obtain data on 236 patients who were admitted to and discharged from a certified public home health agency. Nursing dependency explained a significant amount of variance over and above sociodemographic factors and medical conditions in number of visits made by nurses and length of nursing stay. Measures of nursing dependency should be incorporated into future studies that examine factors related to use of home care nursing resources.

A group of representatives from health, social service, other community agencies and nursing education meet regularly to address issues in providing care to homeless, low‐income, and uninsured persons in Spokane.
