Editor's NoteA year agoPublic Health Nursingpublished special content on home health care (4:2, June 1987). This past year has seen the crisis in home care escalate. Medicare claims denials are soaring. This year there are fewer home care agencies than there were a year ago, particularly voluntary agencies. This means patients, especially those without a payment source, are going without needed care. A recent conversation with a colleague gave rise to the following editorial. It is an eloquent, powerful statement about home care nursing as seen through the eyes of a practicing home care nurse. This guest editorial is adapted from Margaret Myers's “Report to the Subcommittee on Workplace and Salaries of the Governor's Task Force on the Nursing Shortage” for Connecticut. Its message is universal. I invite readers to respond through letters, which might comprise a special section in a future issue.S.

AbstractA retrospective, descriptive study was conducted to gather information on patients receiving high‐tech home care and their families. A newly created data collection form, the health‐specific family coping index, and subjective opinions of primary nurses and nursing administration regarding outcome were used to collect the information. Two groups emerged from the data analysis. One group, labeled successful, encountered few management problems, and consisted mostly of male patients with female care givers. The other group, labeled difficult, had management problems, and were mostly female patients with male care givers. The latter patients were members of younger families in which their illness necessitated role changes within the family. They also had poorer prognoses, more acute illnesses, and longer hospital and community health nursing admissions than the successful group. Family coping scores were also lower in this group, with an average of 28.7 as compared with 40.5 for the successful group. These results appear to be independent of the high‐tech procedure required in the home, which was mastered by most individuals in an average of four nursing visits. AbstractThe purpose of this study was to determine whether replacement of block clinics with integrated clinics improved delivery of service to patients and improved the work environment for staff. To measure the impact of service delivery change, a systematic, random sample of clients was surveyed both before integrating services and one year after integration. A staff survey was also conducted to measure the effects of the change on work performance and satisfaction. Results from the client survey showed significant improvements in average waiting time, perceived shorter waiting time, and use of several services at one visit. Results from the staff survey showed perceived improvements in patient care, more time for patient education and follow‐up, and workload management. Historical ReprintThe annotated bibliography, in edited version, that follows was first published in its entirety 62 years ago in Public Health Nurse; February 18(2):83‐90, 1926. It was first presented as a paper at the Fifth International Congress of Nurses, in Helsingfors, Finland, July 1925. The author, and editor of the journal, Ada M. Carr, acknowledged the assistance of Lavinia Dock, Adelaide Nutting, and A. M. Peterkin. The latter was general superintendent, Queen Victoria's Jubilee Institute for Nurses, London, England. For convenience, Carr divided the literature review into three time periods: 1860‐1900, 1901‐1912, and 1913‐1925. Although she offered the caveat “… this sketch of public health nursing literature cannot in any sense be considered exhaustive,” one cannot help being impressed with the amount and quality of writing published during these years.Ruth N. AbstractThe practice of nurse practitioners (NPs) may change as a result of the perceived oversupply of physicians. Examination of current NP practice is essential to understand how best to manage the future of the profession. This study examined differences in activities and consultation patterns between master's‐prepared and certificate‐prepared NPs. The 1S79 NP students who responded to a nationally mailed questionnaire were surveyed again six months to one year after graduation. A total of 50 activities spanning as many as four client types (adults, infants/children, maternal, family planning) were included in the questionnaire. Participation in general activities for well and ill clients was high for NPs with both types of education. Nurse practitioner activity related to specific health problems decreased as the severity of the problems increased. This was accompanied by increased consultation. In ten activities the difference was significant between the two levels of NP preparation: more master's‐prepared NPs performed six holistic‐oriented activities and more certificate‐prepared NPs performed four technically oriented activities. A significant difference in the numbers of NPs needing heavy consultation was noted for 15 activities; in every case, fewer master's‐prepared NPs required heavy consultation. These results support the emphasis of NP master's degree programs on practice autonomy, especially with well clients. In terms of curriculum, more emphasis should be placed on the care of newborns

AbstractPublic health nurses are actively involved in helping family members cope with stresses in their daily lives. They are looking for research data about methods people actually use to cope with these stresses and their relative effectiveness. This research study was an analysis of the coping responses used by 153 mothers of healthy preschool children when experiencing stress in their everyday lives with the children, the perceived effectiveness of these responses, and factors related to the coping process. Problem‐focused responses were more effective than emotion‐focused responses. The specific responses that were found most effective were handling the problem in small pieces, drawing on past experience, talking the problem over with someone else, not worrying about the problem, and viewing the problem objectively. AbstractThe purpose of this study was to examine the relationship of self‐concept and future orientation to loneliness in a group of pregnant teenagers. Pregnant teenagers attending public health clinics for prenatal care were asked to participate in the study. Subjects completed the Revised UCLA Loneliness Scale, the Heimberg Future Time Perspective Inventory, and the Tennessee Self‐Concept Scale. Data analysis revealed a significant negative correlation between self‐concept and loneliness and a significant negative correlation between future time perspective and loneliness. Stepwise multiple regression analysis indicated that self‐concept and future time perspective together explained 38 percent of the variance in loneliness. The findings revealed that self‐concept was the best predictor of loneliness in this sample and that the prediction of loneliness was strengthened in the presence of a weak future time pe
