摘要:
The growth in the number of people with learning disability living in the community presents a major challenge to primary care services. There is a high prevalence of physical and mental illness among these patients, but disorders may be inadequately managed. This reflects problems of access to primary care services, and problems of liaison between services. Policy options for improving primary healthcare provided to people with a learning disability include: specific monitoring by FHSAs; improved training; clarification of responsibility for medical care; regular medical examinations; and improvements in information systems.

This is an interim report on a study funded for four years by the South East Thames Regional Health Authority to look at the hearing status of adults with severe learning difficulties in the Lewisham and North Southwark District. It follows a preliminary paper published in this journal in 1989, when 100 people had been examined. This second paper gives results up to June 1990, when 300 people had been seen. The number of people with a hearing loss requiring amplification was at least 39.6% of the sample, correlating well with results found in the first group. In this study 32% required objective testing, compared with 19% in the smaller group. The problems associated with such testing are discussed. Sixty‐seven hearing aids were issued by the above date. The necessity of good training and motivation in care staff was found to be essential if aids were to be properly used and scarce resources not wasted. 摘要:
This article is a description of a questionnaire and its results, which was completed with 49 families who have come to live in High Wycombe mainly from Pakistan. They are all caring for someone with a mental handicap, the majority having a number of handicaps. The Community Mental Handicap Team had been aware that many services were not being used by these families and wanted to make the local services more applicable and accessible. One of the main findings was the importance of having a Punjabi speaking member of the Team, who comes from the same cultural group as the clients. This gives the families the confidence to consider which services they would like to use. Another finding was that it is no help to translate information into Urdu as people are generally not literate in Urdu or English. The results highlighted the importance of thinking about cultural issues in all our services such as the right diet and availability of single sex services for young women.

Evaluation of community care only rarely considers management issues and the stresses which arise from new work structures and situations. This study examined the changed role of community care staff and the support required to maintain an effective service. Group interview methods were employed. Factors identified as important stressors were working together in a smaller setting, being in the public gaze and the risk‐taking involved in client training programmes. 摘要:
People with learning disabilities who move out of institutions often remain grouped together, stranded in communities of their own; ordinary social networks can be largely inaccessible to them. In this study of consumer satisfaction with services, people were invited to assess their own lives in terms of their day time occupation, accommodation, leisure, social contacts and the use of services. The main areas of dissatisfaction concerned leisure pursuits and social relationships. A lack of friends and difficulties about going out, often due to a lack of transport, were problems for many people. It seems that service providers do not always take account of social relationships or give them a high enough priority. It was concluded that social skills as well as work skills should be given more attention. In promoting the improvement of people's social lives it is important to consult individuals about what they want. Strategies are suggested for service providers interested in improving social networks.

The aim of this study was to compare the relative efficacy of modelling and self‐instructional training in teaching a group of adults with mental handicaps to complete an abstract task. Modelling here involves demonstrating a task and requesting that a client copy those behaviours he has just observed. Self‐instructional training requires an individual to engage in self‐talk to direct behaviour in a desired direction. The results show little difference in performance on the task between the two groups although individual performance varied. 摘要:
The anti‐epileptic drugs, carbamazepine and sodium valproate, were used to control generalised seizures (tonic clonic) and absence seizures in people with mental handicap in Brock‐hall Hospital, who were not well‐controlled with other drugs like phenobarbitone, phenytoin and primidone. The results indicated that the number of seizures decreased dramatically with these newer anti‐convulsants and without the need for polypharmacy, as was the norm previously.
