An Alcohol Education Service for people with learning difficulties is described. The service addresses the problems of low motivation, the fact that drinking in pubs is a highly valued activity, the low level of knowledge in relation to alcohol, and the psychological and physical sequelae of alcohol abuse. Assessments include: investigations into the reasons why people drink; their knowledge of alternatives to drinking; the nature of alcohol and psychological and physical symptoms; a daily diary recording alcohol intake; and assessment of drinking behaviour in pubs. Treatment sessions include: discussion topics about alcohol; giving clients information about alcohol; review of the psychological and physical symptoms of alcohol; roleplay of various alcohol‐related situations; problem solving tasks; and practice in pubs. Nine Offender patients, who are mentally handicapped and detained in a secure environment were included in a psychotherapy group, which met weekly for sixteen weeks. The patients demonstrated an ability to interact within a group setting and to use the milieu in a therapeutic way. The therapists gained an insight into the dynamics of patient relationships and the subculture of life in a hospital.There was no evidence that running such a group in a secure unit undermined staff authority or caused any deterioration in other staff patient relationships. We would recommend group therapy be included in the range of treatments offered to patients with a mental handicap, including those living in a secure setting.

摘要:
The need to amend the Criminal Procedure (Insanity) Act 1964 has been recognised since the mid 1970s when the Butler Report on Mentally Abnormal Offenders put forward proposals to amend the law. However, successive Governments have failed to give priority to this issue. Following further consideration by the Home Office and pressure from the Law Society and other organisations, a Private Members' Bill was introduced to stop miscarriages of justice to mentally vulnerable people and to end unnecessary detention in psychiatric hospitals. The Bill passed unopposed through all its Parliamentary stages and received Royal Assent on Thursday, 27th June 1991. Given the comments by Home Office ministers during the Parliamentary debates, it is hoped that there will only be a short delay before the new Act is implemented.This paper describes why the New Act was required and how it will benefit people with learning disabilities who are accused of a criminal offence.

This paper reviews the debate surrounding the value of specialised treatment facilities for people with seriously challenging behaviours. Distinctions are made between advantages and disadvantages resulting from specialisation, and those resulting from specalised services being unit based. Distinctions are also made between the inherent disadvantages of services and observed but remediable disadvantages arising from inadequate implementation. Results from a survey of specialised units in England and Wales are presented.

The 1991 Touring Exhibition was organised by Northgate Arts Project (NAP). This article describes the work produced, the reasons for mounting the exhibition and some of the benefits which may accrue from this undertaking. NAP's work to date is described, as well as plans for future expansion. The article is written in the context of current moves to address the matter of arts in healthcare.
